乳腺自动全容积扫查系统与手持超声探头的对比评估 被引量:25

Comparison of automatic breast volume scanning system and hand-held ultrasound

摘要 目的比较乳腺自动全容积扫查系统(ABVS)与手持探头在检出乳腺病变中哪种检查手段更具有优势。方法 187例乳腺疾病患者,先由一位医师(A)用传统手持高频超声探头进行常规乳腺检查,再由另一位医师(B)应用AcusonS2000ABVS系统进行再次扫查并独立阅片。两位同年资的医师分别计数病变的数目并结合美国放射学会制定并为国际采用的乳腺影像及报告系统(BI-RADS)评估指标,对其超声声像图特征进行归纳,并与病理结果相比较,比较两种检查方式的诊断结果。结果 187例患者共187个病变,医师(A)经手持探头检出的病变数目为45个,经病理证实检出率24%(45/187);而医师(B)应用ABVS系统独立阅片后检出的病变数目为63个,经病理证实检出率34%(63/187);二种检查手段在病变的检出率上ABVS优于传统手持探头,且差异有统计学意义(χ2=0.04,P<0.05)。结论 ABVS扫查过程客观,避免人为因素的遗漏,是一种全新的超声检查方法,对简化操作手法及全面扫查等均有较大意义。 Objective To determine which detection method is more advantageous between the automatic breast volume scanning system (ABVS) and hand-held ultrasound in the detection of mammary gland diseases. Methods A total of 187 cases firstly underwent regnlar breast exams by a doctor (A) with traditional hand-held high frequency uhrasound,then by another doctor(B) with Acuson S2000 ABVS system scanning. Doctor B read the pictures independently. The two doctors having the same seniority counted the number of lesions, evaluated lesions according to the international Breast Imaging Reporting and Data System (BI-RADS) which was established by American committee of radiation (ACR), summarized the characteristics of the sonograms, compared with pathological results and compared the diagnostic value of these two methods. Results Totally 187 cases with 187 lesions, doctor A with the hand-held probe detected lesions in 45 [24% (45/187)] lesions and doctor B with ABVS system detected lesions in 63 [34% (63/187 ) ] lesinns. For the detection rate,ABVS is superior to the traditional hand-bald probe. The difference between the two methods was statistically significant (χ^2=0.04, P 〈 0. 05 ) Conclusions ABVS scanning process is objective,avoiding human factors. It is a completely new method of ultrasound examination. This kind of examination will help to simplify the operation technique and scanning.
