游离腓骨瓣移植修复下颌骨缺损8例

Mandibular Defect Reconstruction with Vascularized Fibular Flap： A Report of 8 Cases

目的：探讨游离腓骨瓣在下颌骨缺损修复中的应用价值。方法：选择8例下颌骨缺损患者，利用带血管蒂游离腓骨瓣进行缺损修复，观察修复效果及并发症情况。结果：8例患者所移植骨瓣全部良好成活，术后未见明显血肿、积液等并发症情况。所有患者随访至今，面部外形、健侧咬合情况良好，语言、咀嚼、吞咽功能恢复正常，供区愈合良好．行走正常，未出现踝关节不稳定情况。结论：游离腓骨瓣修复下颌骨缺损效果满意，在恢复患者面部外观及口腔重要生理功能的同时为下领骨病灶切除后义齿的种植修复奠定了基础，在下颌骨缺损修复中将拥有更为广阔的前景。

Objective To study the clinical value of the application of vascularized fibular flap in the mandibular defect reconstruction. Methods 8 patients with mandibular defect were treated with, and the effects and complication was observed after the operation. Results All the fibular flap transplanted were survived, there was no hematoma and hydrops after the operation. The facial appearance and ecelusal function of the healthyside in all the patients was good followed - up to now, language function, masticatoryfunction, and swallowing function get right, the donor site well healed, and able towalk normally, without ankle instability. Conclusion the application of vascularised fibular flap has satisfactory clinical effects in the mandibular defect reconstruction, it can recover the facial appearance and oral physiological function, and lay the foundation for false tooth at the same time. it will have a broad prospect in the mandibular defect reconstruction.
