酒石酸布托啡诺对瑞芬太尼用于全麻病人术后苏醒期的影响

摘要 目的:观察提前静注布托啡诺抑制瑞芬太尼全麻术后苏醒期不良反应的情况及术后镇痛效果的评价。方法:法选择ASAⅠ-Ⅱ级择期上腹部手术患者60例,随机分为三组。每组20例。三组病人于手术结束前30分钟分别给予静脉注射布托啡诺0.02mg/kg(R组)、芬太尼l彬kg(F组)和0.9%生理盐水10ml(D组),记录三组患者苏醒期麻醉恢复时间和躁动程度,以及术后镇痛效果的比较。结果:三组病人的躁动评分:D组病人躁动的发生率高于R、F组(P〈0.05);三组病人的呼吸恢复时间、睁眼时间、拔管时间。R组比F、D组延长(P〈0.05),F组较R、D组苏醒质量好。R和F组的镇痛率均在80%以上。但是R组的副反应如心慌、呼吸抑制较F组少。R组术后恶心、呕吐发生率低于D组(P〈0.05),注药后1hR组头晕和嗜睡的发生率明显高于F组(P〈0.05)。结论:布托啡诺和芬太尼用于全麻术后患者均具有良好的术后镇痛效果,术后不良反应发生率低。而布托啡诺的不良反应较少,且能减少寒战发生,用在术后进入PACU的患者更安全有效。
