前牙种植修复术后美学指数分析

Esthetic index analysis after anterior tooth implantation

目的:观察专科医师对美学区单科种植体的美学效果评估并与患者主观美学满意度相比较。方法:45例在我院接受美学区单颗种植修复的患者通过拍摄口内照片由2名正畸科医师,2名修复科医师,2名牙周科医师,2名种植外科医师,按照红色美学指数(pinka esthetic index,PES)与白色美学指数(whitea esthetic index,WES)标准进行美学评分。同时由患者本人对种植修复美学效果进行视觉模拟评分(visual analogue scale,VAS)。结果:平均PES+WES指数是13.47±0.27,平均PES指数是5.46±0.19,平均WES指数是6.32±0.17。该指标通过Cohen'sκ检验,证实具有良好的前后一致性。Spear man分析显示:PES+WES与VAS具有显著相关性。各组专科医生PES+WES评分经Kr us kal Wal l i s秩和检验显示无统计学差异(P=0.133>0.05),即不同专科医生美学评估结果没有差别;但WES评分具有统计学差异(P=0.019<0.05)。结论:PES+WES作为种植牙美学综合指数能够全面客观地评价种植修复术后的美学效果,可以作为临床科学研究中非常有用的分析工具。VAS分析结果提示临床医生在设计评估前牙单颗种植体时要充分考虑患者对不同牙齿的审美差异。

Objective To observe the implant aesthetic effect in aesthetic area by the specialist and compare with the subjective aesthetic satisfaction by patients. Methods 45 patients who underwent implant prosthesis in aesthetics area in the hospital. Oral photographs were taken to score according to the pink aesthetic index （PES） and white aesthetic index （WES） by two orthodontists,two prosthodontists,two periodontists and two implant dentistry. Degree of patient satisfaction was measured using a visual analogue scale （VAS）. Results The average PES＋WES is 13.47_＋0.27,average PES is 5.46＋0.19, average WES is 6.32＋0.17. The results analysed by Cohen ＂s Kanalysis showed good consistency. Spearman analysis showed that PES ＋ WES and VAS has significant correlation.Each group of dentist PES ＋ WES score by Kruskal Wallis analysis showed no significant differences （P=0.133〉0.05）,that means there were no difference in aesthetic assessment in all dentists;and WES score has statistically difference （P=0.019〈0.05）. Conclusion PES＋WES as an aesthetic index can evaluate the aesthetic effect of implant,and can be used as a clinical analysis tooI.VAS results indicate the implant design should be considered the patients＂ aesthetic differences.
