甲胎蛋白异质体对肝细胞癌早期诊断的临床意义

Clinical Signficance of Lectin-reactive Alpha-fetoprotein in Early Detection of Hepatocelluiar Carcinoma

目的探讨甲胎蛋白异质体(AFP-L3)在肝细胞癌(HCC)早期诊断中的临床应用价值。方法采用微量离心柱法分别分离42例HCC、27例肝硬化、34例病毒性肝炎患者血清中的AFP-L3,通过化学发光法检测血清AFP及AFP-L3含量,并进行比较分析。结果以AFP-L3≥10%为阳性诊断标准,HCC患者血清中AFP-L3异常升高明显高于肝硬化组及病毒性肝炎组(P<0.01),灵敏度为64.72%,特异度为95.16%,诊断准确度为88.06%。结论 AFP和AFP-L3在HCC的诊断中具有较好的互补性,联合诊断有利于HCC的早期诊断。

Objective To investigate the application value of AFP-L3 in early detection of liver cell car- cinoma （HCC）. Methods Using micro centrifugal column, AFP-L3 were isolated from 42 patients with HCC,27 cases of liver cirrhosis, 34eases of patients with viral hepatitis. By chemiluminescence assay, serum AFP and AFP-L3 of these patients were tested,then a comparative analysis was made. Results AFP-L3 ≥ 10% is regarded as the diagnostic criterion. The abnormal elevation of AFP-L3 serum level in HCC patients was significantly higher than that of cirrhosis and viral hepatitis group （ All P 〈 0.01 ）, the sensitivity, speci- ficity and accurate diagnosis rate was respectively 64.72% ,95.16% and 88.06%. Conclusion In the ear- ly diagnosis of HCC,AFP and AFP-L3 has good complementarity,combined application is more conducive.
