口腔颌面部软组织炎性肌纤维母细胞瘤21例临床分析

Clinical analysis of 21 cases with inflammatory myofibroblastic tumor of oral and maxillofacial soft tissues

目的:探讨口腔颌面部炎性肌纤维母细胞瘤的病因、临床表现、鉴别诊断和治疗。方法:回顾分析中国医科大学附属口腔医院1990-2011年收治的21例炎性肌纤维母细胞瘤(包括炎性肌纤维母细胞瘤的其他名称)的临床资料,总结其临床症状、病灶区的影像学表现、病理学表现、临床治疗方案以及术后随访情况。结果:21例病例均在病灶边界外0.5～1 cm切除病灶。10例随访5 a以上,6例随访3 a,3例随访2 a,其中3例复发。对于复发病例,采用低剂量分次放射治疗。1例随访3 a,肿物明显减小,1例随访2 a,肿物基本消失,1例随访半年,无明显变化。结论:口腔颌面部炎性肌纤维母细胞瘤确诊依赖组织病理学表现,其中手术治疗为首选治疗方法。若入路困难或者多次复发,可辅助放射或激素治疗。

PURPOSE： To explore the pathogenesis and clinical characteristics and treatment of inflammatory myofibroblastic tumor in oral and maxillofacial region.METHODS： Twenty-one cases with inflammatory myofibroblastic tumor of oral and maxillofacial soft tissues were reported.Their clinical features,radiological and pathological manifestations,treatment and prognosis were retrospectively analyzed with related literature review.RESULTS： Twenty-one patients were treated by surgery.Three recurrences were found in 19 cases during following up.2 cases was lost follow-up.Ten in 21 patients had been followed up for 5 years.6 patients followed up about 3 years.3 patients followed up about 2 years.3 cases were treated by radiotherapy for postoperative recurrence.One in 3 relapsed cases had been followed up for 3 years and the tumor size decreased.one patient followed up about 2 years.The tumor disappeared 2 months after treatment.One patient was followed up about half year.No change in tumor size.CONCLUSION： Diagnosis is depend on biopsy for inflammatory myofibroblastic tumor of oral and maxillofacial soft tissues.Surgery is a primary and effective treatment method.If surgical incision cannot be accessible,radiotherapy or hormonal therapy can be used as an adjunctive therapy.
