双针法在老年患者肺穿刺活检术中的应用价值及安全性评估

Clinical application and safe analysis of double needle percutaneous transthoracic biopsy in elderly patients with pulmonaryle mass

目的探讨CT引导下双针法经皮肺穿刺术对老年患者胸部结节性疾病的诊断应用价值及其安全性评估。方法回顾性分析本院36例老年患者在CT引导下双针法经皮肺穿刺活检术的资料,分析该方法对胸部结节性疾病诊断的准确率及其安全性评估。结果第1针穿刺取材成功率72.2%,第2针穿刺取材成功率100%,病灶穿刺成功率100%;36例穿刺活检的病理诊断结果与后续治疗和随访结果吻合。双针法肺穿刺术并发症的总发生率为27.8%(10/36);,依次为气胸11.1%(4/36),肺出血19.4%(7/36),痰中带血8.3%(3/36)。结论 CT引导下双针法肺穿刺活检术对老年胸部结节性疾病的诊断是一种准确性高、安全可靠又简便易行的诊断方法。

Objective To explore the clinical application and safe analysis of double needle percutaneous transthoracic biopsy in elderly patients with lung mass. Methods The intact data of 36 elderly patients under going percutaneous pulmonary biopsy with doule biopsy needle under CTS guidance were analyzed. The diagnoses of all the cases were confirmed by pathology and follow up. Results One sev enty two lung biopsies were performed on 36 patients and satisfactory tissue specimens obtained in all patients, with the successful rate of 100%. The diagnoses for malignant tumors and benign lesions were 34 cases and 2 cases respectively. The overall accuracy rate was 100%. There were 10 cases of postoperative complications （27.8%）, of which 4 cases of pneumothorax （ 11.1% ） , 7 cases of pneumor rhagia（ 19.4% ） and 3 cases of hemoptysis seen （8.3%）. Massive hemorrhage did not occur in all the patients. Conclusion CTS guided percutaneous transthoracic biopsy with double biopsy needle is an accurate, safe and quick diagnosis method for pulmonaryle mass in elderly patients.
