血液灌流干预联合血液透析在终末肾衰竭并发高血压患者中的临床研究

Clinical studies of hemoperfusion invention patients with terminal renal hypertension

目的探讨血液灌流干预联合血液透析在治疗终末期肾衰竭并发高血压的临床治疗疗效。方法 94例终末肾高血压患者的临床资料随机均分为对照组与观察组。对照组(n=47)接受充分血液透析治疗每周3次,不少于12h,同时已接受慢性肾衰一体化治疗,联合、足量应用降血压药物。观察组(n=47)在此基础上加用活性碳灌流器每月进行血液灌流治疗2h/次。研究时间为16个月,对两组治疗后的平均收缩压、平均舒张压、PRA、ATⅡ以及PTH、治疗后患者的生存质量进行对比。结果两组患者治疗后的平均收缩压、平均舒张压、PRA、ATⅡ以及PTH值比较,差异有统计学意义(P<0.05)。根据患者生存质量评分标准得知,观察组治疗后的生存质量得分为(133±5)分,明显高于对照组生存质量得分(98±6)分,两者比较差异有统计学意义(P<0.05)。结论血液灌流联合血液透析治疗终末肾患者高血压的临床疗效显著,应该在临床上加以推广应用。

Objective To investigate blood perfusion intervention combined with hemodialysis in the treatment of end-stage renal failure hypertension clinical therapy. Methods 94 cases of patients with terminal renal hypertension were divided into two parts：the control group and the observation group.The control group was accepted the full dialysis treatment three times per week not less than 12 hours,has accepted the integration of treatment of chronic renal failure,jointly,the application of sufficient quantities of blood pressure lowering drugs.The observation group（n=47）was accepted plus monthly hemoperfusion with activated charcoal perfusion treatment of 2 hours/times. Study period of 16 months,the two groups after treatment on this basis. The average systolic blood pressure,mean diastolic blood pressure that PRA,AT H and PTH,after treatment, the patient＇s quality of life were compared. Results After treatment,the average systolic blood pressure, mean diastolic blood pressure of PRA,AT II and PTH values with statistically significant differences（P 〈 0.05）.Based on the patients quality of life score that the quality of life score in the observation group （133 ± 5）points,significantly higher than the quality of life score（98 ±6）points,both statistically significant differences（P 〈 0.05）. Conclusion The clinical efficacy of hemoperfusion combined with hemodialysis treatment of terminal renal patients with hypertension,should be promoted and applied in clinical practice.
