天门市第一人民医院呼吸内科住院患者抗菌药物应用分析

Use of Antibacterials in The Patients from The Department of Respiratory Medicine in Tianmen Municipal First People's Hospital

目的:调查我院呼吸内科住院患者抗菌药物的应用情况,为促进临床合理用药提供依据。方法:采用回顾性调查研究方法,对我院呼吸内科2010年7—12月542例使用抗菌药物住院患者的病原学检测和抗菌药物的使用种类、用药频度(DDDs)、药物利用指数(DUI)、联合用药及不合理用药情况等进行统计分析。结果:我院呼吸内科住院患者抗菌药物的使用率为94.3%(542/575),共涉及7类15个品种;联合用药率为26.9%(146/542),病原学送检率为91.7%(497/542),阳性率为14.9%(74/497)。DDDs排序居前3位的抗菌药物分别为美洛西林、头孢哌酮/舒巴坦钠、左氧氟沙星。结论:我院呼吸内科抗菌药物使用基本合理,但仍存在一些问题,应进一步提高合理用药水平。

OBJECTIVE：To investigate the antibacterial use in the patients from the department of respiratory medicine of our hospital in order to promote clinical rational administration of antibacterials. METHODS： In this retrospective study, a total of 542 patients who used antibacterial drugs from July to December in 2010 were analyzed statistically with regard to the etiological test, the types of antibacterials used, the DDDs, DUI （ drug utilization index）, combined use and irrational use of antibacterials. RESULTS ： A total of 7 types of antibacterials involving 15 kinds were used in 94. 3% （542/575） of the inpatients reviewed; combined use of antibiotics was noted in 26.9% （ 146/542 ） ; etiological test was performed in 91.7 % （497/542） with positive rate of 14. 9% （74/497）. Mezlocillin, cefoperazone / sulbactam sodium and levofloxacin ranked among the top 3 in DDDs. CONCLUSION： The use of antibacterials in the inpatients from the department of respiratory medicine of our hospital was basically rational yet there are still some problems. The rational antibiotic use level remains to be further improved.
