通络滋肾汤治疗ⅢA型前列腺炎的疗效观察

Efficacy of Collateral-dredging and Kidney-nourishing Decoction for Type ⅢA Prostatitis

目的:观察通络补肾法治疗ⅢA前列腺炎的临床疗效。方法:选择2011年6月—2012年2月在中国中医科学院西苑医院泌尿外科门诊就诊的ⅢA型前列腺炎患者67例,按照随机抽样法分成2组,其中治疗组36例,对照组31例。对照组患者服用盐酸坦洛新缓释胶囊,每次0.2 mg,早晚各1次,同时服用甲磺酸左氧氟沙星片每次0.2 g,早晚各1次。治疗组在应用对照组治疗方案的同时,联合应用通络滋肾汤,每次1袋,每袋200 mL,早晚各1袋。2组的观察周期均为1个月。结果:对照组失访2例,治疗组失访3例。治疗后,治疗组的总有效率为87.88%(29/33),对照组的总有效率为65.52%(19/29),2组比较差异有统计学意义(P<0.05)。治疗后,治疗组患者的慢性前列腺炎症状积分指数(NIH-CPSI)都有明显改善,差异均有统计学意义(P<0.05);治疗后,对照组患者的NIH-CPSI都有明显改善,差异均有统计学意义(P<0.05);2组治疗后NIH-CPSI比较,其中患者的疼痛或不适症状评分、排尿症状评分、生活质量评分、总积分方面,治疗组明显优于对照组,差异均具有统计学意义(P<0.05)。治疗后,2组前列腺液常规检查中白细胞计数和治疗前比较,都有明显改善,差异有统计学意义(P<0.05);2组间比较,差异无统计学意义(P>0.05)。结论:通络滋肾法治疗ⅢA型前列腺炎效果较好,可较大程度地缓解患者的疼痛或不适症状、排尿症状以及改善患者的生活质量,值得临床推广应用。

OBJECTIVE ： To evaluate the efficacy of Collateral-dredging and Kidney-nourishing Decoction for type ⅢA prostatitis. METHODS： 67 patients with type ⅢA prostatitis who were seen in the outpatient department of urinary surgery of Xiyuan Hospital of China Academy of Chinese Medical Sciences were randomly assigned to either treatment group （ n = 36） or control group （ n = 31 ） to be treated with Tamsulosin Hydrochloride Sustained Release Capsules （0. 2 rag） Nocte et mane plus Levofloxacin Mesylate Tablets （0. 2 g） Nocte et mane; however, the treatment group received add-on Collateral-dredging and Kidney-nourishing Decoction （one pouch of 200 mL Nocte et mane）. Two groups were followed for a period for one month. RESULTS： The control group lost in 2 cases, the treatment group 3 cases dropped out. The total effective rate was 87.88% （29/33） for treatment group as compared with 65.52% （ 19/ 29） for the control group, and the differences between the two groups were statistically significant （P 〈 0.05 ）. After treatment, the NIH-CPSI symptom scores in both the treatment group and the control group showed significant improvement （P 〈 0. 05 ） ; however, the treatment group was significantly better than in the control group in pain or discomfort symptom score, voiding symptom score, quality of life score and total score of NIH-CPSI （P 〈 0. 05）. After treatment, both the treatment group and the control group showed significant improvement in WBC （white blood count） as observed in prostate fluid routine examination as compared with before treatment （ P 〈 0.05 ） , however, the differences between the two groups were not statistically significant （P 〉 0. 05 ）. CONCLUSION ： Treatment of type m A prostatitis with Collateral-dredging and Kidney-nourishing Decoction resulted in satisfactory curative efficacy for it can greatly alleviate patient＇ s pain symptoms or malaise and voiding symptoms and improve patient＇ s quality of life, hence it is worthy of clinical application.
