大肠癌患者手术前后外周血淋巴细胞的变化及其临床意义

The Changes of Peripheral Blood Lymphocytes and Its Clinical Significance in Colorectal Cancer Patients before and after Operation

目的:探讨大肠癌患者手术前后外周血淋巴细胞的变化及其临床意义。方法:采用流式细胞术(FCM)检测50例正常对照者和54例大肠癌患者手术前后外周血淋巴细胞水平。结果:①大肠癌组与正常对照组比较,大肠癌患者外周血CD4+细胞水平显著下降,CD8+细胞水平显著上升,CD4+/C D8+严重倒置,NK细胞水平升高,CD1 9+细胞水平显著下降,差异均有统计学意义(P<0.05);CD3+细胞水平无变化,差异无统计学意义(P=0.417)。但大肠癌术前和术后比较,除CD19+细胞水平显著下降且差异有统计学意义(P=0.000)外,其余指标间差异均无统计学意义(P>0.05);②不同临床分期的大肠癌患者,Ⅰ～Ⅱ期与Ⅲ～Ⅳ期比较,除CD19+细胞水平显著降低且差异有统计学意义(P<0.05)外,其余指标均无明显变化,差异均无统计学意义(P>0.05);③有淋巴结转移的大肠癌患者和无转移的比较,NK细胞和CD19+细胞水平显著升高,差异有统计学意义(P<0.05);其余指标均无明显变化,差异均无统计学意义(P>0.05)。结论:大肠癌患者免疫功能降低;CD19+细胞水平与大肠癌临床分期可能有关,NK细胞和CD19+细胞水平与大肠癌有无淋巴结转移可能有关。

Objective To investigate the peripheral blood lymphocytes and its clinical significance in patients with colorectal cancer before and after surgery.Methods The level of peripheral blood lymphocytes before and after flow cytometry（FCM） detection of 50 cases of normal controls and 54 patients with colorectal cancer surgery.Results （1）A large colorectal cancer group and normal control group comparison, colorectal cancer in patients with peripheral blood CD4＋ cell levels of significant decline of CD8＋ cells in the level of significant with the rise,of CD4＋/CD8＋ serious inversion,of NK cell level rise,of CD19＋ cell level significantly with the decline,the differences were statistically significant（P 〈 0.05）;CD3＋ cell level did not change,the difference was not statistically significant（P=0.417）.Colorectal cancer patients before and after surgery, in addition to the CD 19＋ cells were significantly decreased and the difference was statistically significant（P=0.000）,and the remaining indicators the difference was not statistically significant（P 〉 0.05）;（2）In different clinical stages of colorectal cancer patients, Ⅰ-Ⅱ,Ⅲ-Ⅳin addition to the level of CD19＋ cells was significantly lower and the difference was statistically significant（P 〈 0.05）,and the remaining indicators did not ctmnge significantly, the difference There was no significant（P 〉 0.05）;（3）Lymph node metastasis in patients with colorectal cancer and non-transfer of NK cells and CD19＋ cell level was significantly increased,the difference was statistically significant（P 〈 0.05）;remaining indicators did not change significantly, no sigmficant difference（P 〉 0.05）.Conclusion colorectal cancer pataents with reduced anmune function;of CDI9 ceils and colorectal cancer clinical staging may be relevant, NK cells and CD19＋ cell levels and colorectal cancer lymph node metastasis may be relevant.
