摘要
目的探讨抗环瓜氨酸肽抗体(CCP)、类风湿因子(RF)、超敏C-反应蛋白(HS-CRP)、抗核抗体(ANA)在类风湿性关节炎诊断中的应用。方法抗CCP抗体采用ELISA法检测,HS-CRP和RF采用速率散射比浊法检测,ANA采用间接免疫荧光法检测。结果在RA患者中HS-CRP阳性检出率为83.9%,RF的阳性检出率为77.9%,抗CCP抗体阳性检出率为75.6%,抗ANA抗体最低仅为33.7%。RF的敏感性和特异性分别为77.9%、76.7%,抗CCP抗体的敏感性和特异性分别为75.6%、98.9%,HS-CRP敏感性和特异性分别为83.9%、71.7%,抗ANA抗体的敏感性和特异性分别为33.7%、45.6%。结论抗CCP抗体有较高的敏感性和很高的特异性。多项血清学指标联合检测有助于提高类风湿性关节炎的临床诊断的特异性。
Objective Of anti-cyclic eitrullinated peptide antibodies ( CCP), rheumatoid factor ( RF), high-sensitivity C-reactive protein (HS-CRP), antinuelear antibody (ANA) in rheumatoid arthritis diagnosis. Methods Anti-CCP antibodies by ELISA, HS-CRP and RF using rate nephelometry assay, ANA using indirect immunofluoreseenee. Results In the HS-CRP in RA patients positive rate 83.9%, RF positive detection rate of 77.9%, anti-CCP antibodies was detected in 75.6%, the lowest anti-ANA antibody only 33.7%. RF sensitivity and specificity were 77.9% and 76. 7%, anti-CCP antibodies, the sensitivity and specificity were 75. 6%, 98.9%, HS-CRP sensitivity and specificity were 83.9%, 71.7%, anti-ANA antibody sensitivity and specificity were 33. 7%, 45. 6%. Conclusion Anti-CCP antibodies have high sensitivity and high specificity. Joint detection of muhiple serum markers help to improve the specificity of the clinical diagnosis of rheumatoid arthritis.
