Relationship between laryngeal neoplasm and human papilloma virus

摘要 目的 :研究 HPV与喉上皮性肿瘤的关系。方法 :采用一对 HPV L1区通用引物及 6、1 1、1 6、1 8型特异 E6区引物多聚酶链反应 ,对 1 2 8例喉癌及喉上皮增生性病变样本进行 HPV检测。结果 :2 4例检出HPV,1 6例为 HPV6型 ,4例 HPV 1 1型 ,1例 HPV 1 8型 ,2例未定型。 60例喉癌中 5例阳性 ,其中 3例 6、1 1型均为整合状态 ,另 2例为未定型。结论 :大部分喉癌和上皮增生性病变与 HPV感染无关 ;良性型 HPV可整合于宿主细胞核 ;对少数 HPV阳性患者 ,除采用手术、化疗及放疗外 ,还需抗病毒治疗。 Objective:To investigate the relationship between the laryngeal neoplasm and human papilloma virus(HPV).Methods:The 128 cases with formalin fixed paraffin-imbedded,of laryngeal squamous cell carcinoma(LSCC) and laryngeal epithelium hyperplastic lesion(LEHL) were detected for HPV infection by PCR with HPV consensus L 1 primer and 6,11,16,18 type specific E 6 primers.Results:The presence of HPV DNA was detected only in 5 of 60 LSCC,11 of 15 laryngeal papillomas,4 of 10 dysplasia and 1 of 10 keratosis.None HPV was detected in 15 laryngeal polyps and 9 adjacent chronic laryngitis.Most of HPV found were benign types,except that one keratosis was HPV18.Conclusions:This study suggests that most of LSCC and LEHL are not associated with HPV infection.Benign HPV types also integrate with host cell genes in malignant lesions.Besides surgery,chemotherapy and radiation therapy,antiviral therapy should be accepted in a few HPV positive patients.
