摘要
目的观察老年早期糖尿病肾病(DN)患者外周血单个核细胞中核因子-κB p65的磷酸化活化表达水平及血清炎症因子超敏C反应蛋白、肿瘤坏死因子a、白介素-6、白介素-1β水平,探讨NF-κB表达水平与胰岛素抵抗、尿白蛋白排泄率、糖脂代谢等因素的关系。方法以2型糖尿病早期肾病老年患者(DN组)66例,老年健康体检者对照组(NC组)30例为研究对象,测定炎症指标:外周血NF-κB p65的磷酸化水平、血清炎症因子水平,生化指标、胰岛素抵抗指数,以及观察DN组尿白蛋白排泄率。结果 DN组外周血NF-κB p65水平和血清炎症因子超敏C反应蛋白、肿瘤坏死因子、白细胞介素-6、白细胞介素-1β水平比NC组升高。相关分析显示,NF-κB与FBG、GHB、HOMA-IR、UAER正相关。结论老年早期糖尿病肾病患者外周血NF-κB表达水平升高,胰岛素抵抗、尿白蛋白排泄率、血糖水平影响其活性,其中尿白蛋白及高血糖可升高NF-κB的水平。
Objective To observe the early elderly patients with diabetic nephropathy outside individual nuclear cells nu- clear factor-KB p65 phosphorylation of activation expression, serum inflammation factors allergic e-reactive protein, tumor necrosis factor a, interleukin 6, interleukinβ 1 level, to explore the relationship of nf-kappa B expression level and insulin resistance, urinary albumin excretion rate, sugar lipid metabolism of the factors. Methods Collected type 2 diabetes early elderly patients with kidney disease (DN group) 66 cases, old age the healthy check-up control group (NC group) 30 cases as the research object, the determination of inflammation index: peripheral blood the nf-kappa B p65 phosphorylation, serum level of inflammation factors level, biochemical index, calculate insulin resistance index, and observe urinary albu- min excretion rates DN group. Results The results of peripheral blood DN nf-kappa B p65 level and serum inflammation factors allergic c-reactive protein, tumor necrosis factor, interleukin 6, interleukin-β1 level than NC group was increased. Related analysis showed that, NF-~B and FBG, GHB, homa-ir, UAER were related. Conclusion Elderly patients with diabetic nephropathy peripheral blood NF-~B expression levels rise, its expression level and blood sugar control level, insulin resistance, urinary albumin excretion rates.
