大分割与常规分割放疗对恶性肿瘤骨转移癌痛的疗效观察

Observation on pain relieving effect of big fraction and conventional fraction radiotherapy for bone metastasis

目的观察大分割及常规分割放疗对骨转移癌痛的疗效。方法 43例恶性肿瘤骨转移患者被随机分为两组,大分割放疗组21例,300 cGy/次,每周5次,DT:3 000 cGy,常规分割放疗组22例,200 cGy/次,每周5次,DT:4 000cGy。结果全组放疗止痛总有效率为81.4%,大分割放疗组止痛有效率为80.95%,常规分割放疗组止痛有效率为81.82%,两者疗效无显著性差异(P>0.05)。放疗1周内上痛起效率大分割组为71.43%,常规分割组为72.73%,两组比较无显著差异(P>0.05)。结论放疗是治疗骨转移癌痛的有效方法,不同的放疗分割方式有效率和放疗1周内止痛起效率无显著差异,可以根据患者的情况,如病情、预后及经济情况等来选择。

Objective To observe the pain relieving effect of big fraction and conventional fraction radiotherapy for bone metastasis. Methods Forty three patients with bone metastasis were randomly divided into two groups, 21 cases received big fraction radiotherapy in the way of DT ： 30 Gy, 5 fractions per week, 3 Gy per fraction, and 22 cases received conventional fraction radiotherapy in the way of DT： 40 Gy, 5 frac- tions per week, 2 Gy per fraction. Results The pain relief rate was 81.4% in the whole group,The pain relief rate was 80.95 % in the big frac- tionated group and 81.82% in the conventional group. There was no significant difference between the two different fractionation scheme for Bone metastases pain relief（ P 〉0.05 ）. After radiation for 1 week, the rate of efficiency for pain relief in big segmentation group was 71.43%, it in conventional segmentation group was 72.73 %, there was no significant difference between these two groups （ P 〉 0.05 ）. Conclusion Radiothera- py is an effective way to relieve pain in patients with bone metastasis. The different fractionated styles of radiotherapy for bone metastasis can be de- termined by the condition of patients such as status of illness, expected life span, economic condition of the family and so on.
