乳腺癌改良根治术后局部复发的临床研究

Analysis of Local Recurrence after Radical Mastectomy in Breast Cancer

目的探讨乳腺癌改良根治术后局部复发患者的有效治疗方案,以延长其生命,提高其生活质量。方法回顾性分析新乡市第一人民医院乳腺科2002年12月至2006年12月期间收治的37例乳腺癌改良根治术局部复发患者,研究其临床病理特征及治疗手段,胸壁复发24例,腋窝淋巴结复发4例,锁骨上淋巴结复发9例;37例患者均行全身化疗,28例胸壁及腋窝淋巴结复发患者再次手术后化疗,32例患者接受放疗。结果局部控制30例,占83.3%;未控制7例,占26.7%。单发病灶与多发病灶控制率有显著性差异(P<0.05)。随访5年,单纯手术组与综合治疗组5年生存率有显著性差异(P<0.05)。结论乳腺癌改良根治术局部复发患者积极再次手术可大大提高局部控制率,采取综合治疗方案可提高患者的生存率,延长其无病间期。

Objective Explore an optimal treatment for local recurrence after radical mastectomy. Methods 37 patients with breast cancer recurrence who had undergone radical surgery in Xinxiang first people＇s hospital from January 2002 to January 2006 were retrospectively analyzed. Analysis their clinicopathological characteristics and treatment. Results The follow-up the 37 patients, local control rates 83.3% （30/37）, local uneontrol rates 26.7% （7/37）. There was a statistical difference in multi-site recurrence and simple-site recurrence between different subgroups （P〈0.05）. There was a statistical difference insingle treatment and combined treatment program （P〈0.05）. Conclusion Local radiotherapy combined with chemotherapy and surgery is a better mode to improve the local control rate. Forrecurrent breast cancer, combined treatment program can improve the survival rate.
