创建“112”双向转诊模式:中国双向转诊的难点与对策研究

Developing the "112 two-way referral mode" to study the difficulties of two-way referral mode in China and its countermeasures

一直以来,我国双向转诊普遍存在的问题是上转容易而下转难,要让更多的患者有社区首诊也很难。要解决这一难题不能只是依赖行政干预,而是要更多地依靠体制和机制创新,"112"双向转诊模式正是根据多年的医改理论研究和实践体会,从体制和机制创新为切入点和着力点而提出的双转诊新模式。"112"双向转诊模式为:建立由现有公立医院与众多公立基层卫生机构成为"一家人"的双向转诊管理体制;建立一套合理、有效的双向转诊奖励和问责机制;建立公立与非公立两种城乡医院集团的公平双向转诊竞争机制。也许只有正确、规范地运用"112"双向转诊模式,才能真正实现"小病在社区、大病进医院、康复回社区"的目标,才能更有效地解决城乡居民的看病难问题。

The two-way referral mode in China has a common problem,which is difficult to transfer patients from superior hospital to primary hospital and keep more patients in community health centers.To solve this problem,system and mechanism innovation are needed,rather than administrative intervention.Based on theoretical research and practical experience in many years,the new ＂112 two-way referral mode＂ was developed to achieve system and mechanism innovation,which included the management system closely combining public hospitals with many primary health centers,reasonable and effective two-way referral reward and accountability mechanism,and fair competition mechanism between public and private hospitals.It is thought that,only by application of the ＂112 two-way referral mode＂,can the goal of ＂ailment in community,serious illness in hospital and rehabilitation back to community＂ be achieved,and the problem of ＂Difficult Medical Treatment＂ be effectively solved.
