产吲哚黄杆菌脑脓肿一例

Brain abscesses caused by Chyrseobacterium indologenes:report of one case

摘要 患者男,66岁。2012年1月初开始出现头痛、恶心。起病后约3d出现左侧肢体乏力,起病后6d左侧肌力减退到1度,意识模糊。影像学检查发现右侧顶枕部病灶(图1),疑为“脑梗死”进行治疗,在使用“皮质激素”和“脱水治疗”1周后,左侧肌力恢复到4~5度、意识状态好转后出院。出院后2d,肌力再次减退到1~2度并出现明显的定向能力障碍,复查MHI发现右侧顶枕部多发囊性病灶(图2),拟“脑脓肿”,使用甲硝唑、头孢他啶和磷霉素三联药物进行经验性抗感染治疗,症状无明显好转,住院2周后转入我院。
