活动式自粘性俯卧垫在经皮肾镜取石术中的应用价值研究

Study of application on the movable and self-adhesion prone position cushions in percutaneous nephrolithotomy surgery

目的探讨活动式自粘性俯卧垫在经皮肾镜取石术(PCNL)体位护理中的应用效果。方法选取2009年6月～2011年12月于我院进行PCNL手术治疗的90例患者为对照组,术中进行常规的体位处理,同期选取90例患者为观察组,术中采用活动式自粘性俯卧垫进行体位摆放,后将两组患者的手术时间,患者舒适度,并发症发生率,麻醉前,麻醉后10 min,术中30、60、120 min的呼吸循环指标、血清皮质醇、血糖水平进行比较。结果观察组的手术时间短于对照组,舒适度优于对照组,并发症发生率低于对照组,围术期的呼吸循环指标、血清皮质醇、血糖水平波动幅度均小于对照组,差异均有统计学意义(均P<0.05)。结论活动式自粘性俯卧垫可显著改善PCNL术患者的舒适度,降低术中体位对患者呼吸循环和应激方面的影响。

Objective To study the application effect of movable and self-adhesion prone position cushions in the position nursing of patients treated with percutaneous nephrolithotomy surgery （PCNL）. Methods 90 patients treated with PCNL surgery in our hospital from June 2009 to December 2011 were selected as control group, they were intervened with routine position management, and 90 patients at the same time weres elected as observation group, they were intervened with po- sition management by movable and self-adhesion prone position cushions, then the surgery time, comfort degree, incidence of complications and respiratory and circulatory indexes, serum cortisol, blood glucose before anesthesia and after anesthe- sia at 10 min, during operation at 30, 60 and 120 min of two groups were compared. Results The surgery time was shorter than that of control group, comfort degree was better than that of control group, incidence of complications was lower than that of control group, fluctuation range of respiratory and circulatory indexes, serum cortisol, blood glucose during perioper- ative period were all smaller than those of control group, there were all significant differences （all P 〈 0.05）. Conclusion The movable and self-adhesion prone position cushions can significantly improve the comfort degree of patients treated with PCNL surgery, and it can decrease the influence of position for the respiratory and circulatory, stress degree of pa- tients during perioperative period.
