Evaluation of left ventricular systolic dysfunction in patients with type 2 diabetes mellitus by speckle tracking imaging

摘要 目的:探讨斑点追踪成像(STI)技术评价2型糖尿病(T2DM)患者左心室收缩功能的早期改变。方法:选取77例LVEF正常的T2DM患者,依据有无合并高血压分为单纯糖尿病组(DM组)39例,糖尿病合并高血压组(DH组)38例,正常对照组(NC组)34例,应用STI技术测量左心室长轴切面的纵向收缩期峰值应变(SLs)、短轴切面的圆周(SCs)及径向收缩期峰值应变(SRs)。结果:左心室整体SLs在两病例组显著降低(P<0.01),其中DH组降低更加显著(P<0.01);左心室整体SCs及SRs仅在DH组显著降低(P<0.01)。结论:无心力衰竭症状LVEF正常的T2DM患者的左心室收缩功能已降低。 Objective: To assess the early changes of left ventricular (LV) systolic function in patients with type 2 diabetes meUitus (T2DM) by speckle tracking imaging (STI). Methods: Seventy-seven T2DM patients with normal left ventricular ejection fraction (LVEF) were divided into 2 groups according to their blood pressure : T2DM isolated group ( DM group, n = 39) and T2DM associated with hypertension group ( DH group, n = 38 ). Thirty-four healthy individuals were enrolled as normal control (NC) group. Standard two dimensional images were acquired from the LV apical and short-axis views. LV systolic peak longitudinal strain (SLs), systolic peak circum- ferential strait/ (SCs) and systolic peak radial strain (SRs) were measured by STI. Results: The average SLs of global LV was significantly lower in both DM and DH groups than that in control group ( P 〈 0. 01 ), while it was significantly more prominent in DH group (P 〈 0. 01 ). The average SCs and SRs of global LV were only significantly reduced in DH group (P 〈 0. 01 ). Conclusion: Left ventricular systolic function had significantly impaired in T2DM patients with normal LVEF without symptomatic heart failure.
