乳腺癌患者外周血T细胞亚群与NK、NKT细胞检测的临床意义

The clinical significance of detecting T cell subsets,NK & NKT in peripheral blood of breast cancer patients

目的:检测乳腺癌患者外周血T淋巴细胞亚群及NK、NKT细胞表达率的变化,探讨乳腺癌患者免疫状况与肿瘤分期的关系以及免疫指标变化在治疗前后的临床价值。方法 :采用荧光标记法与多参数流式细胞仪检测40例正常女性和120例乳腺癌患者的总T淋巴细胞(CD3+)、T辅助/T诱导细胞(CD3+CD4+)、T抑制/细胞毒细胞(CD3+CD8+)、自然杀伤细胞(NK)、NK样T细胞(NKT)的表达率,并进行统计学分析。结果:无淋巴结转移组乳腺癌患者治疗前CD3+、CD3+CD4、CD3+CD8+、CD4+/CD8+与对照组比较无统计学意义,NK、NKT显著低于对照组(P<0.05)。淋巴结转移组乳腺癌患者CD3+、CD3+CD4、CD3+CD8+、NK、NKT表达率均明显低于对照组(P<0.05)。经过综合治疗后乳腺癌组各项免疫指标均恢复到正常值。结论:外周血T细胞亚群的检测对判断乳腺癌患者的病情、临床分期有一定意义;经过治疗后能够有效地改善乳腺癌患者的免疫状态。

Objective： To study the change of expression rate of T lymphocyte subsets, NK ＆ NKT cell in peripheral blood of breast cancer patients, and find out the relationship between immune status of breast cancer patients and neoplasm staging, and its clinical value of immune indices before and after the therapy. Methods： By fluorescent marker method and multi - parameter flow cytometry, 40 nor- mal females and 120 females with breast cancer were tested for the expression rate of CD3 ＋ , T helper/T induced cell CD3 ＋ CD4 ＋ , T sup- pressor/cytotoxic ceils CD3 ＋ CD8 ＋ , NK and NKT, and then the results were analyzed with statistic method. Results ： Compared with data of control group, the expression rates of CD3 ＋, CD3 ＋ CD4 ＋ , CO3 ＋ CO8 ＋ and Cl）4 ＋ ~CO8 ＋ ratio in breast cancer patients without lymph node metastasis had no obvious difference, while the expression rates of NK and NKT were significantly lower than that of control group （ P 〈 0. 05 ） . The expression rates of CD3 ＋ , CD3 ＋ CD4 ＋ , CD3 ＋ CD8 ＋ , NK and NKT in breast cancer patients with lymph node me- tastasis group were significantly lower than that of control group （ P 〈 0. 05 ） . But the immune indices returned to normal standard after a comprehensive treatment. Conclusion ： The detection of T cell subsets in peripheral blood is an effective way in the diagnosis of breast cancer and its clinical stages. The therapy can strengthen the immunity of breast cancer patients.
