加速康复外科理念在腹腔镜下子宫肌瘤剔除术中的作用 被引量:14

Observation of fast track surgery in patients with laparoscopic myomectomy

摘要 目的探讨加速康复外科(FTS)理念和措施在腹腔镜下子宫肌瘤剔除术患者围手术期的安全性和有效性。方法选择2010年10月至2011年10月青岛市市立医院妇科因子宫肌瘤行腹腔镜下子宫肌瘤剔除术的患者116例,分别采用加速康复外科和传统的围手术期处理原则,比较两组患者术后恶心呕吐的发生、术后肛门排气时间、住院时间、住院费用以及手术并发症。结果两组患者均治愈出院。加速康复组患者与传统组相比,术后恶心、呕吐的发生、术后肛门排气时间、住院时间以及治疗费用明显低于对照组,差异有统计学意义(P<0.05)。FTS理念的应用未增加手术并发症。结论将加速康复外科理念用于腹腔镜下子宫肌瘤剔除术患者围手术期处理,有利于患者的康复,缩短住院时间,降低住院费用,且不会增加手术并发症。 Objective To observe the feasibility and safety of fast track surgery in patient with laparoscopic myomectomy. Methods 116 patients undergoing laparoscopic myomeetomy were randomly divided into two groups:fast track group and conventional group. The operative time, blood loss, the postoperative nausea and vomiting, recovery of bowel function, post- operative hospital stay, medical cost and perioperative complications were compared. Result There were no significant differences in operative time and blood loss between two groups. The postoperative nausea and vomiting, postoperative hospital stay and medical cost in fast track group patients were significantly lower, and recovery of bowel function was fas- ter than those in conventional group, P 〈 0. 05. FTS didn't increase the perioperative complications. Conclusions Fast track surgery is more favorable in guiding the perioperative management of laparoscopic myomectomy.
