血脂水平与体外受精周期胚胎质量的相关性初步研究

Research on the Association between the Blood Lipid Level and the Embryo Quality of IVF/ICSI

目的:研究不孕症患者血脂水平与体外受精(IVF/ICSI)胚胎质量的相关性。方法:对行IVF/ICSI治疗、符合纳入标准的646例不孕症患者的资料进行回顾性分析,测定、分析血清三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)、非高密度脂蛋白(nonHDL)、低密度脂蛋白(LDL)和脂蛋白(a)[LP(a)]水平与胚胎质量的关系。血脂水平和IVF/ICSI胚胎质量相关性采用Spearman相关分析。结果:患者年龄、体质量指数(BMI)、基础FSH、窦卵泡数、Gn用量及血脂水平临床妊娠组和非临床妊娠组之间均无统计学差异;受精率、卵裂数、优质胚胎数和优质胚胎率临床妊娠组均显著高于未临床妊娠组(P<0.05)。血脂水平和IVF/ICSI胚胎质量相关分析显示:TG和TC与卵子数、受精数及卵裂数均显著负相关,TC、LDL及nonHDL与优质胚胎数呈显著负相关(P<0.05),其余均无统计学意义。结论:血脂水平临床妊娠组与未临床妊娠组间均无统计学差异;IVF/ICSI胚胎质量与TC、LDL及nonHDL呈显著负相关。

Objective： To investigate the association between the blood lipid level and the embryo quality of IVF/ICSI. Methods： A total of 646 patients of infertility who received the IVF/ICSI in the department of reproductive medicine were recruited to the research. The embryo outcome, the lipid level including triglycerides （TG）, total cholesterol （TC）, high-density lipoproteins （HDL）, non high-density lipoproteins （non HDL）, low-density lipoprotein （LDL）, lipoprotein （a） [LP（a）] were researched. Spearman correlation was performed to analyze the correlation between lipid level and embryo quality. Results： Statistical analysis revealed that between the clinical pregnant group and non-pregnant group there was no significantly difference in age, BMI, basic FSH, AFC, the dosage of Gn and the lipid level. The fertilization rate, cleavage number, high-quality embryo number and rate of clinical pregnant group were higher than those of the non-clinical pregnant group （P〈0.05）. Spearman correlation analysis revealed that the level of serum TG and TC negatively correlated with the embryo number, fertilization number and cleavage number. TC, LDL and nonHDL negatively correlated with the high-quality embryo number （P〈0.05）. Conclusion： Between the clinical pregnant group and non-pregnant group there is no significantly difference in the lipid level. The levels of serum TC, LDL and nonHDL negatively correlate with the embryo quality.
