神经肌肉电刺激治疗运动神经元病患者吞咽功能障碍的护理

Nursing of Neuromuscular Electrical Stimulation on Patients with Dysphagia following Motor Neuron Disease

目的探讨神经肌肉电刺激治疗运动神经元病(motorneurondisease,MND)患者吞咽功能障碍的疗效及护理方法。方法按随机数字表法将2009年4-12月于河北以岭医院肌萎缩专科住院的运动神经元病伴吞咽功能障碍患者60例分为对照组和观察组各30例,对照组进行常规药物及传统康复治疗,观察组在对照组的基础上配合神经肌肉电刺激,疗程1个月,比较两组患者的疗效。结果经过1个月治疗,两组患者的吞咽功能障碍电视透视检查(video fluoroscopy swallowing study,VFSS)评分均有所提高,但观察组较对照组评分更高(P<0.05);根据洼田饮水试验评定,观察组治疗后的疗效优于对照组,差异有统计学意义(P<0.05)。结论神经肌肉电刺激可显著改善运动神经元病吞咽功能障碍患者的吞咽功能,有利于提高患者的生活质量。

Objective To investigate the curative effect and nursing countermeasures of neuromuscular electrical stimulation（NMES） on patients with dysphagia following motor neuron disease（MND）. Methods From April ro December 2009, 60 patients who were diagnosed as dysphagia were randomly divided into treatment group（n=30） and control group（n=30）. The patients in both groups received routine medicine treatment and conventional rehabilitation treatment, and the treatment group underwent the additional NMES. The curative effect of two groups were compared after a month. Results After one month＇s treatment, the scores of video fluoroscopy swallowing study （VFSS） were significantly elevated in both groups and the scores were significantly higher in the treatment group than those in the control group（P〈0.05）. Meanwhile, the therapeutic effect in the treatment group was significantly superior to that in the control group according to the assessment of water swallow test（P〈0.05）. Conclusion NMES can be an effective treatment for dysphagia of patients with MND and improve their quality of life.
