内镜下尼龙绳套扎及透明帽辅助黏膜切除术治疗消化道大息肉及平坦型息肉49例的临床体会 被引量:2

摘要 目的:探讨内镜下单环尼龙绳套扎及透明帽辅助高频电切除(endoscopic mucosal resection cap,EMRC)消化道大息肉及平坦型息肉的临床价值。方法:采用经内镜下活检证实为消化道息肉49例58枚息肉,46枚为有蒂大息肉均行尼龙绳套扎联合高频电切除,12枚为平坦型息肉,其中10枚抬起征阳性,行内镜透明帽辅助黏膜切除治疗,2例单发平坦息肉非抬起征阳性,行手术治疗。结果:本组49例58枚息肉中56枚一次性完全切除,完全切除率96.55%(56/58),2例单发平坦型息肉非抬举征阳性者转外科手术治疗,术后病理均为腺癌。完全切除病灶再次病理检查53枚息肉均为良性病变,其中20枚伴轻度异型增生,3枚为腺瘤性息肉顶部部分癌变。4枚息肉术中出血,经内镜止血成功,未发生穿孔、感染等并发症。1月后复查内镜,所有病例创面愈合,3例腺瘤部分癌变者随访延迟12个月无复发。结论:内镜下尼龙绳套扎及透明帽辅助高频电切除消化道大息肉及平坦型息肉是一种创伤小,切除完整,安全有效的方法。
