腹腔镜甲状腺次全切除术52例临床疗效观察

The Clinical Curative Effect Observation of Laparoscopic Subtota Thyroidectomy in 52 Cases

目的探讨腹腔镜甲状腺次全切除术的临床应用价值.方法回顾性分析2009年3月至2012年5月昆明医科大学第二附属医院腹部微创外科完成的52例腹腔镜甲状腺次全切除术的手术患者方法及临床效果.结果 52例患者均顺利完成手术,术中无大出血及中转开放,胸前皮肤红肿、瘀斑5例,声音嘶哑3例,吞咽疼痛2例,头晕、呕吐5例,瘢痕增生1例,术中、术后均无死亡病例.手术时间53～150 min,术后第3～7天出院.结论虽然腹腔镜甲状腺切除术操作困难,但安全可行,其具有明显的美容效果、创伤小、恢复快等优点,有技术条件的腔镜中心可选择合适的病例开展.

Objective To explore the value of clinical application of laparoscopic subtotal thyroidectomy.Methods 52 patients in Department of Abdominal Minimally Invasive Surgery of the Second Affiliated Hospital of Kunming Medical University were treated with laparoscopic subtotal thyroidectomy from March 2009 to May 2012,and the methods and clinical effects were reported.Results 52 patients were successfully completed surgery,and no intraoperative massive hemorrhage and interim open operation occurred.There were 5 cases of chest skin redness,swelling and bruises,3 cases of hoarseness,2 cases of Swallowing pain,5 cases of Dizziness and vomiting,and 1 case of Scar tissue hyperplasia,and no cese of death during or after operation.The operation time was 53～150 min,and all patients were recovered after 3～7 days.Conclusion Although laparoscopic subtotal thyroidectomy is a difficult operation,it is safe and feasible.It has obvious effect of beauty,small trauma,quick recovery and other advantages,the laparoscope center with enough technology conditions can develop the operation in appropriate cases.
