烧伤患者尿FDP、LZM的变化及临床意义

摘要 本文观察了44例四组不同烧伤面积患者的尿FDP(fibrin degradation)、尿LZM(Lysozyme),并分析了与伤情、败血症或死亡的关系。测试结果显示:随着伤情的增重尿FDP、LZM上升幅度逐渐增高,10%以下烧伤面积尿FDP、LzM均在正常范围,分别为0.24μg/ml、4.61μg/ml,而C、D组尿FDP上升为1.65μg/ml、3.5μg/ml;尿LZM30.5μg/ml、99.24ug/ml,两者呈正相关,而且与尿蛋白、尿素氮的上升水平也呈正相关。尿FDP上升1.248ug/ml,尿LZM上升51μg/ml以上提示肾功能有衰竭情况,特别是尿FDP上升4.992,尿LZM上升91μg/ml以上标志予后不佳。本检测对判断病情,评价治疗效果,予示肾功情况、败血症的发生与病情转归有一定临床价值,而且较敏感。 This paper reported the relationship between the levels of urine FDP and lysozyme and the clinical state in burn patients. The patients were devided into 4 groups according to their burn surface, i. e. group A (burn surface less than 10%), group B (11-30%), group C (31-50%,) and group D (51-96%). The levels of urine FDP and lysozyme gradually increased, in proportion to the severities of the burn, for example the levels keeping in normal range in group A but increased markedly in group C or D (1.65 or 3.5ug/ml and 30.5 or 99.24 ug/ml respectively). Another interesting phenomenon is that the prognosis of burn patients was very poor when the levels of FDP or lysozyme reached to 4.99 ug/ml or 91 ug/ml respectively. Urine FDP and lysozyme detection seems to be of value in estimating the severity of burn and predicting the renal functions and the presence of septesis following burn injury.
