下肢骨折不愈合12年治疗护理观察分析

目的:从作者自身感受出发,对车祸致左股骨中段骨折、骨不愈合治疗护理分析,总结经验教训,以促进病人早日康复。方法:受伤后第一次经急诊行股骨钢板螺钉内固定术,第二次行股骨骨折不愈合取髂骨植骨钢板螺钉内固定术,第三次行股骨骨折腓骨移植取髂骨植骨钢板螺钉内固定术,并髋人字石膏固定术,第四次行股骨骨牵引术,小夹板固定,中药治疗。结果:第一次和第二次手术未见骨痂生长,骨不愈合。第三次手术骨痂生长慢,术后8年取出内固定物后三个月再次原位股骨骨折,第四次行股骨骨牵引并小夹板外固定,中西药治疗,两年后骨折完全愈合。结论:骨折治疗过程中要及时纠正贫血和补钙,加强营养,正确功能锻练,切记过早负重,并指导病人自我护理,才能保证骨折的顺利愈合和受伤肢体的康复。

Objective： Starting from the authorg own experience of the car accident, this paper analysis the treatment of nursing care to the left upper middle period of the femoral fractures, summarizes the experiences, for promoting patients recover. Methods： The first operation was done as emergency treatment by femoral screw steel plate fixation after the injury. The second operation was done by femoral screw steel plate fixation with autologous iliac bone struet. The third operation was done by femoral screw steel plate fixation with autologous iliac bone struct and taking fibular transfer, fixing with gypsum covering the hip externally as treating. The third opera- tion was done by continuous skeletal traction of femur, using fixation with small splint and traditional Chinese medicine at the same time. Results ： of the fourth operations are nonunion after the first and second operation, femur callus slow growth and femoral fracture again eight months later after the third operation, and the femoral fracture healing completely two years later after the fourth operation. Result： In the treatment of fractures , it needs suppling calcium and correcting anemia correctly, and increasing nutrition, proper functional exercise, avoiding early weight training, and instructing patient to self - care, for the fracture healing and injured body recovery.
