Necessity and Faced Problems of Drgs Implementation in Public Hospitals

"看病难,看病贵"已成为当今我国社会一个重要的民生问题,有效控制医疗费用的增长,合理使用卫生信息资源,使全民享有低费用、高质量的医疗卫生服务,长期保持医疗卫生事业的持续健康发展是一个世界性的难题。诊断相关疾病组(Diagnosis Related Groups,DRGs)被认为是解决这一问题的重要工具之一。在公立医院改革试点的新形势下,分析DRGs在公立医院实施的必要性,并阐述了病案首页中诊断应采用ICD-10编码,首页信息详细准确;组织专家编写常见病、多发病临床路径,并试点使用和推广常见病、多发病临床路径;制定出各疾病分组的出院标准,推进患者"一卡通"服务,完善患者的就诊信息化管理;鼓励医疗技术的创新。

The short of high-quality medical resources is becoming an important problem of the people＇ s livelihood in our country. Each nation in the world are exploring how to control effectively the growth of medical cost, use rationally the health infor- mation resource, provide high quality but low cost medical service, and keep health service to develop continually and healthily. Diagnosis related groups（ Diagnosis Related Groups, DRGs） are considered an important solution to this problem. In order to ex- plore the reformation experiment of public hospitals, this paper evaluated the necessity of DRGs in a public hospital, and described the canonical medical records which were in line with ICD-10. The clinical pathway of commonly encountered diseases and fre- quently-occurring diseases were prepared and generalized. The disease subgroups discharge standard was developed. The medical service card was used widely, and the disease information of patients was managed by computers. The medical technology innova- tions are encouraged in our hospitals.
