子宫颈部人乳头瘤病毒感染状况调查及基因亚型分析 被引量:2

摘要 目的调查泰安地区妇女子宫颈部人乳头瘤病毒(HPV)感染及基因亚型的分布情况,为该地区HPV亚型流行病学研究及宫颈癌防治提供理论依据。方法选门诊及住院疑似HPV感染的宫颈炎患者675例,用专用宫颈刷采取子宫颈脱落细胞,提取DNA,用基因芯片技术检测HPV23种亚型。结果HPV感染阳性者189例,感染率为28.00%。HPV阳性患者中高危亚型感染145例,占76.72%,低危亚型感染44例,感染率为23.28%。高危亚型感染率排在前6位的依次是16型(57例次,占21.96%)、52型(27例次,占9.93%)、66型(18例次,占6.62%)、33型(17例次,占6.25%)、58型(16例次,占5.88%)、18型(12例次,占4.41%);低危亚型以43型(44例次,占16.18%)、11型(33例次,占12.13%)为主。单一基因型感染129例,占感染人数的68.25%;双重感染42例,占感染人数的22.22%;多重感染18例,占感染人数的9.52%。结论泰安地区妇女宫颈HPV感染率较高,以单一亚型感染为主,HPV16、43、11、52、66是泰安地区HPV感染的主要亚型。利用基因芯片技术对育龄女性进行HPV感染筛查及早治疗,并对持续高危亚型感染和混合亚型感染患者进行重点跟踪是防治宫颈癌的关键。 Objective To investigate the infection situation and sub-genopype distribution of human papilloma virus (HPV) in female subjects in Taian area. Methods Exfoliated cells from 675 female subjects were harvested with specific cervical brush and DNA samples were prepared for gene chips analysis of 23 genotypes of HPV. Results Totally 189 samples were HPV infected and the percentage of positive HPV was 28.0%. 18 subtypes of HPV were screened. High risk subtypes and low risk subtypes of HPV infection rates were 76.72% (145 cases) and 23.28% (44 cases) , respectively. Data also showed that the primary six high risk subtypes were HPV-16 (21.96%), HPV-52 (9.93%), HPV-66 (6.62%), HPV-33 (6.25%), HPV-58 (5. 88% ) and HPV- 18(4.41% ). The two main low risk subtypes of HPV were HPV-43 (16.18%) and HPV-11 (12.13%). 129 cases (68. 25% ) were single genotype infection; 42 cases (22. 22%) and 18 cases (9.52%) were double genotypes infection and multiple genotypes infection, respectively: Conclusions Among female population in Taian area, the infection rate of HPV is 28.0% and the primary infection manner is single genotype infection. The primary five high risk sub-geontypes are HPV-16, 52, 66, 33 and 58. HPV infection screening and early-stage treament of female subjects employing gene chip methods, as well as follow=up on high risk sub-genotypes infected and mixed genotypes infected patients,are the key points of cervical cancer prevention.
