复方南星止痛膏配合颈椎牵引治疗颈型颈椎病30例

30 Cases of Cervical Spondylopathy Treated by Nanxingzhitong Plaster and Cervical Traction

目的探讨复方南星止痛膏治疗颈型颈椎病的临床疗效与安全性。方法 28例采用颈椎牵引治疗(A组),27例采用颈椎牵引加布洛芬缓释胶囊治疗(B组),30例采用颈椎牵引加复方南星止痛膏治疗(C组),均持续治疗7d。分别采用视觉疼痛模拟评分法(VAS评分)与皮尺测量法对治疗前、治疗后3、7天以及停止治疗后14天的疼痛进行评分与颈椎活动度进行测量。结果 3组VAS评分在治疗后7d与治疗前相比皆有差异(P<0.05),B组、C组与A组在治疗后7dVAS评分之间有差异(P<0.01),B组、C组优于A组;B组与C组在治疗后7dVAS评分之间无差异(P>0.05)。A组在停止治疗后14dVAS评分与治疗前相比无差异(P>0.01),B组、C组在停止治疗后14dVAS评分与治疗前相比有差异(P<0.01),与治疗后7d相比皆无差异(P>0.05)。3组在治疗后7d颈椎活动度在各个方向上数值与治疗前相比皆有差异(P<0.05),A组与B组在治疗后7d颈椎活动度在各个方向上数值相比无差异(P>0.05),C组与A组、B组在治疗后7d颈椎活动度在各个方向上数值相比皆有差异(P<0.05);A组在停止治疗后14d颈椎活动度在各个方向上数值与治疗前相比无差异(P>0.05),B组、C组在停止治疗后14d颈椎活动度在各个方向上数值与治疗前相比皆有差异(P<0.05),与治疗后7d相比皆无差异(P>0.05),但C组仍优于B组(P<0.05)。未发现复方南星止痛膏有任何毒副作用。结论颈椎牵引加复方南星止痛膏治疗颈型颈椎病起效较快;安全、有效、方便,值得在临床上推广应用。

OBJECTIVE To investigate the efficacy and safety of Nanxing Zhitong Plaster on the treatment of cervical spondylopathy.METHODS From October 2009 to March 2011,85 cases with cervical spondylopathy were divided into group A（28 cases）,group B（287cases） and group C（30 cases）,among which 48 cases were male and 37 cases were female,ranging in age from 39 to 58 with an average of 47.3.Their course of disease was from 1 week to 32 weeks.The patients in group A were treated with cervical traction;group B were treated with cervical traction and ibuprofen sustained release capsules;group C were treated with cervical traction accompanied with Nanxing Zhitong Plaster.Therapeutic duration were 7 days.The pain and the cervical range of motion were measured by visual analogue scale scores（VAS scores）and tape measurement before and after treatment（on Day 3,7 and 14 days after treatment,respectively）.RESULTS There were significantly differences in the levels of VAS scores in the 3 groups on the 7th day after treatment compared with pretreatment（P0.05）.There were also significantly differences in the levels of VAS scores between group A and group B on the 7th day after treatment（P0.05）.Group B was better than Group A.There were significantly differences in the levels of VAS scores between group A and group C on the 7th day after treatment（P0.05）.Group C was better than Group A.There were no different in the levels of VAS scores between group C and group B on the 7th day after treatment（P0.05）.There was no different in group A in the levels of VAS scores at the 14th day after treatment compared with pretreatment（P0.05）.There was significantly different in group B and C in the levels of VAS scores at the 14th day after treatment compared with pretreatment（P0.05）.There was no different in the levels of VAS scores in group C and group B on the 14th day after treatment compared with the 7th day after treatment（P0.05）.Compared with the beginning,cervical range of motion in all directions showed significant difference at the 7th day after treatment in the three groups（P0.05）.There was no different cervical range of motion in all directions between group A and B at the 7th day after treatment（P0.05）.There were significantly different cervical range of motion in all directions between group C and A or group C and B on the 7th day after treatment（P0.05）.Compared with the beginning,cervical range of motion in all directions showed no difference on the 14th day after treatment in group A（P0.05）.Compared with the beginning,cervical range of motion in all directions showed significant difference on the 14th day after treatment in group B and C（P0.05）.Compared with the 7th day after treatment,cervical range of motion in all directions showed no difference on the 14th day after treatment in group B and C（P0.05）;however,group C was still better than group B（P0.05）.No adverse effect of Nanxing Zhitong Plaster was found in the observation.CONCLUSION Cervical traction and Nanxing Zhitong Plaster has a quick effect on the treatment of cervical spondylopathy.It is effective,safe and convenient,worthy of being widely used.
