痰热清注射液辅助抗生素降阶梯治疗老年重症肺炎的临床观察

Clinical observation of Tanreqing injection combined with de-escalation therapy of antibiotics in elderly patients with severe pneumonia

目的探讨痰热清注射液辅助抗生素降阶梯治疗老年重症肺炎的临床效果。方法选择60例老年重症肺炎患者,将患者分为观察组和对照组,每组30例。对照组采用抗生素降阶梯治疗,观察组在对照组的基础上辅用痰热清注射液,观察两组临床症状消退时间、治疗效果和免疫功能变化。结果观察组的咳嗽消退时间、肺部啰音消失时间和退热时间明显早于对照组,P<0.05。观察组治疗总有效率为96.67%,显著高于对照组的80.00%,P<0.05。观察组CD4+、CD8+明显高于对照组,而CD4+/CD8+显著低于对照组,P<0.05。结论痰热清注射液辅助抗生素降阶梯治疗老年重症肺炎临床效果显著,改善免疫功能。

Objective To observe the clinical effects of Tanreqing injection combined with de-escalation therapy of antibiotics in elderly patients with severe pneumonia. Methods 60 cases of elderly patients with severe pneumonia selected from October 2010 to Feb- mary 2012 were randomized to the observation group and control group （n = 30 cases） by using random number table. The control group was treated with de-escalation therapy of antibiotics, and the observation group was given Tanreqing injection on the basis of the control group. The subsided time of clinical symptoms, the clinical effects and immune function were observed in two groups. Results The sub- sided time of coughing, pulmonary rules and fever in the observation group were significantly earlier than those in the control group （ P 〈 0. 05 ）, The total effective rate in the observation group was 96.67% , which was significantly higher than that （80. 00% ） in the control group （P 〈 0. 05 ）. CD4＋ and CD8＋ in the observation group was significantly higher than those in control group （P 〈 0. 05 ）, while CD4＋ / CDs＋ was significantly lower than those in the control group （ P 〈 0.05 ）. Conclusion Tanreqing injection combined with de-escalation therapy of antibiotics in elderly patients with severe pneumonia had singificant effect and significantly improved immune function.
