吡嗪酰胺治疗糖尿病合并肺结核对肾功能影响分析

Effect of Pyrazinamide on renal function as in the treatment of diabetes with pulmonary tuberculosis

目的观察吡嗪酰胺治疗糖尿病合并肺结核时对患者肾功能的影响,指导临床用药。方法选取2010年1月～2012年1月我院收治的160例糖尿病合并肺结核患者,分别于服用吡嗪酰胺前、服用后1个月对肾功能进行监测。结果患者服用吡嗪酰胺前后血肌酐、尿素氮较用药前均无显著变化(P>0.05),但尿微量白蛋白(P<0.05)、血清胱抑素C(P<0.05)、血尿酸(P<0.01)含量较用药前明显升高,差异具有统计学意义。结论吡嗪酰胺可显著影响肾脏对尿酸的排泄,肾功能敏感指标亦可出现异常,用药期间应注意监测,以防出现不良损害。

Objective To observe the effect of pyrazineamide on renal function as in the treatment of diabetes with pulmonary tu- berculosis and to guide clinical medication. Methods 160 patients of diabetes with pulmonary tuberculosis from January 2010 to January 2012 in our hospital were selected and they received pyrazineamide. The renal function was monitored before and one month after taking pyrazineamide. Results There were no significant changes in the serum creatinine （Ser） and blood urea nitrogen （BUN） before and af- ter using pyrazineamide （ P 〉 O. 05 ）. But microalbuminuria （ P 〈 0.05 ） , serum eystatin C （ P 〈 0.05 ）, blood uric acid （ P 〈 O. 01 ） con- tents were obviously higher after using pyrazineamide than those before taking pyrazineamide, with significant differences. Conclusion Pyrazineamide can significantly affect the excretion of uric acid in the kidney, renal sensitive index may also appear unusual, and it should take attention to monitoring to avoid bad damage.
