肌力训练对痉挛型脑瘫患儿步行功能的影响 被引量:6

摘要 目的观察肌力训练对痉挛型脑瘫患儿下肢肌力及步行功能的疗效。方法将51例痉挛型脑瘫患儿随机分为观察组26例及对照组25例,观察组行肌力训练治疗,对照组采用常规康复训练治疗,治疗6周。采用徒手肌力测定法测定两组治疗前(T0)、治疗后即刻(T1)、治疗6周(T2)下肢肌力(踝跖屈肌、膝伸直肌、髋伸直肌),改良Ashworth评定量表对观察组肌张力进行评定,采用GMFM中D和E功能区对两组不同时点进行步行功能评定;采用足印法进行步态分析,计算步速。结果观察组T1、T2时点膝伸直肌和髋伸直肌肌力明显高于同组T0时点及对照组,T1、T2时点GMFMD、GMFME评分明显高于同组T0时点及对照组,T1、T2时点步行速度明显高于同组T0时点及对照组,P均<0.05。余指标无统计学差异。结论肌力训练能促进痉挛型脑瘫患儿下肢肌力及步态功能的改善,并不增加肌张力。
