和胃方治疗糖尿病胃轻瘫临床对照研究

目的:对比中西药疗效,寻求糖尿病胃轻瘫的有效治疗方法。方法:60例随机分为中药组及西药组各30例。治疗前记录患者的症状评分,并行13C-辛酸呼气试验法测定胃固体半排空时间。中药组给予自拟和胃方口服,西药组给予多潘立酮,疗程4周。对两种疗法进行对比。结果:两组症状均改善(P<0.01),胃固体排空速度均加快(P<0.01)。中药组胃排空速度改善优于西药组(P<0.01),胃轻瘫症状总体改善也优于西药对照组(P<0.05),尤其上腹痛、排便异常和脘腹胀满、泛酸改善优于西药组(P<0.01和P<0.05)。结论:和胃方可有效改善胃轻瘫症状,提高胃排空速度,效果优于传统促动力药多潘立酮。

Objective：To investigate the efficacy of Chinese Herbals and Western medicines and seek the effective treatment for Diabetic Gastroparesis.Methods：The total 60Patients,diagnosed as Diabetic Gastroparesis,who match the enrolled conditions,were randomLy divided into two groups：Observation group（traditional Chinese drugs group,n=30）;Comparison group（western medicine group,n=30）.For there were no significant differences in gender,ages,blood glucose levels and durations of Diabetes,the two groups have comparability as expected.Chinese medicine observation group：given self-made oral Chinese medicine He Wei-fang 1Payment each,2 times daily,100mL every time,for 4 weeks;Western medicine control group：received domperidone a half hourPro-meals orally,1Piece each,3 times daily,for 4 weeks.Record thePatient＇s symptom scores and detect the solid gastric half-emptying time with 13C-octanoic acid breath test method,bothPre-treatment and 4 weeksPost-treatment.Result：the symptoms of diabetic gastroparesis were improved（P0.01） and Solid gastric emptying rate were increased（P0.01） in both groups.But the improvement of gastric emptying rate in the Chinese Medicines group was better than that in western medicine group（P0.01）,meanwhile,the overall improvement of symptoms of gastroparesis was also better than that in the comparison group（P0.05）,especially,the symptoms such as upper abdominalPain,abnormal bowel movements and abdominal distention,pantothenic acid were improved evidently,compared to domperidone group（P0.01,P0.05）.Conclusions：Evidently improved the symptoms of diabetic gastroparesis and gastric emptying rates,He Wei-fang,which was better than the traditionalProkinetic drug domperidone,could be a feasible approach for the treatment of diabetic gastroparesis and have obvious clinical significance.
