腹腔镜技术在妇科良恶性肿瘤中的应用分析

目的:探讨腹腔镜技术在妇科良恶性肿瘤治疗中的临床应用价值。方法:回顾性分析2010年2月～2011年12月间我妇科收治的56例妇科良恶性肿瘤患者的临床资料,根据患者意愿分为两组,其中观察组20例应用腹腔镜诊治,对照组36例采用传统开腹术诊治。观察比较两组的手术时间、麻醉时间、术中出血量、恢复进食时间、术后住院天数及术后并发症发生等情况。结果:所有患者手术全部成功。观察组20例术中因胆囊炎症粘连严重,解剖结构辨别不清转开腹治疗2例;因子宫肌瘤巨大并发生变性转开腹1例;因术中诊断为卵巢癌中转开腹1例。其余16例均在腹腔镜下完成手术。两组患者手术时间、麻醉时间、术中出血量、恢复进食时间、术后住院天数比较差异显著,观察组显著优于对照组(P<0.05)。除对照组3例术后发热引发上呼吸道感染外(经抗感冒药物后治愈),两组患者术后切口无出血和感染,均恢复良好。平均随访1.5年,除对照组1例发生切口疝外,其余均无严重并发生。结论:应用腹腔镜治疗妇科良恶性肿瘤的疗效显著,具有损伤小、痛苦轻、术后恢复快、安全有效、并发症少等优点,可作为治疗妇科良恶性肿瘤的重要手段之一。

Purpose：Laparoscopic techniques discussed in gynecology benign and malignant tumor of clinical application value.Methods：Retrospective analysis in February 2010 to December 2011 were between I gynecology 56 cases of gynecological cancer patients is the clinical data,according to the patient will divided into two groups,including the observation group 20 cases of application of laparoscopic make a diagnosis and give treatment,the control group with traditional open surgery 36 cases of diagnosis and treatment.Study the curative effect of two.The results：All patients all successful operation.The observation group curative effect is significantly better than the control group（P 0.05）.The observation group the postoperative complications have significantly less than the control group（P0.05）.Conclusion：Application of laparoscopic treatment of gynecological tumors curative effect is distinct,and can be used for treatment of gynecological malignancy is one of the important methods.
