94例甲状腺手术中超声刀的应用 被引量:11

摘要 目的探讨超声刀在开放性甲状腺手术中的临床应用。方法收集安徽医科大学第二附属医院耳鼻咽喉头颈外科2009年1月~2012年6月完成的甲状腺手术186例,其中超声刀手术(超声刀组)94例、传统手术(传统组)92例。比较两组的手术时间、术中出血量、切口长度、术后引流量和术后并发症情况。结果超声刀组手术时间为(60.23±0.97)min,传统手术组为(95.12±1.35)min,平均缩短34.89min,两组比较差异有统计学意义(P<0.05),超声刀组术中出血量为(16.81±0.63)ml,传统手术组为(51.80±0.64)ml,平均减少34.99ml,两组比较差异有统计学意义(P<0.05),超声刀组手术切口长度(4.33±0.06)cm,传统手术组为(9.45±0.11)cm,平均缩短5.12cm,两组比较差异有统计学意义(P<0.05),术后引流量、术后并发症比较差异均无统计学意义(P>0.05)。结论应用超声刀行开放性甲状腺手术安全可靠,可明显缩少手术时间,减少术中出血量,缩小手术切口。 Objective To investigate the value of ultrasonic-harmonic scalpel in open hyperthyroidism thyroidectomy. Methods From January,2009 to June,2012,186 patients were treated with thyroidectomy by the same operation team,including 94 cases receiving UHS thyroidectomy(UHS group) and 92 cases undergoing conventional thyroidectomy(the conventional group).The operation time,intraoperative bleeding volume,the length of incision,postoperative draining volume and postoperative complications were compared between the two groups. Results The mean operating time in the UHS group was(60.23±0.97)min,which was 34.89min in mean shorter than that in the conventional group(95.12±1.35)min.The mean quantity of intraoperative bleeding volume in the UHS group was(16.81±0.63)ml,34.99ml less than that in the conventional group(51.80±0.64)m.The mean length of incision in UHS group was(4.33±0.06) cm,5.12 cm smaller than that in the conventional group(9.45±0.11) cm.No statistically significant difference were found in draining and postoperative complications between the two groups(P0.05). Conclusions Compared with conventional method,the use of UHS in thyroidectomy is safer and more effective.It can reduce the operation time and the intraoperative bleeding.
