摘要
目的通过分析糖尿病患者尿液中的微量清蛋白、免疫球蛋白G、α1微球蛋白和β2微球蛋白的含量变化,探讨四项尿微量蛋白定量检测对糖尿病肾病的诊断价值。方法健康组(n=62)为健康体检者,疾病组(n=81)为糖尿病患者,疾病组根据糖化血红蛋白水平不同又分为疾病A组(HbA1c<7%,n=35)和疾病B组(HbA1c≥7%,n=46),所有受检对象均在外院做过四项尿微量蛋白定量检测,回顾分析检测结果。结果通过分析检测结果,疾病A组与疾病B组的四项尿微量蛋白含量均高于健康组,差异有统计学意义(P<0.05),疾病B组的含量均高于疾病A组,差异有统计学意义(P<0.05);疾病B组患者尿微量蛋白联合检测的阳性率分别为:微量清蛋白联合α1微球蛋白为62.8%、微量清蛋白联合免疫球蛋白G为68.1%、微量清蛋白联合β2微球蛋白为70.2%;四项联合的检测阳性率为92.6%,四项联合检测的阳性率明显高于两个指标联合检测,差异有统计学意义(P<0.05)。结论糖尿病患者定期检测尿微量蛋白有助于早期发现肾损害,四项尿微量蛋白联合检测的阳性率较高,值得临床推广。
Abstract Objective Through the analysis of the diabetic urine trace albumin,immunoglobulin G, alpha 1 small globulin and be- ta 2 micro globulin variation in content of, this paper discusses four urine trace protein quantitative detection of diabetic nephropathy diagnostic value. Methods Health group (n=62)for the healthy check - up, disease group ( n = 81 )for patients with diabetes, disease group glycosylated hemoglobin levels according to different and divided into disease group A( HbA1 c 〈 7%, n = 35 )and disease group B ( HbA1 c quartile 7 %, n = 47 ), all participants in the outer court client had four urine trace protein quantitative detection, review test results. Results Through the analysis of the test results, the group A disease with disease group B urine trace protein content of the four were higher than healthy group, P 〈 0.05 ), the content of group B disease are higher than the disease, A group ( P 〈 0.05 ) ; Disease group B patients trace protein urine joint test positive for respectively:trace albumin joint α1 small globulin is 62.8% ,trace albumin joint immunaglobulin G for 68.1% ,trace albumin joint beta 2 micro globulin is 70.2% ;Four joint test positive rate was 92.6% ,four joint test positive two index was significantly higher than the joint test( P 〈 0. 05 ). Conclusion Diabetes patients periodically test urine protein helps to trace found early renal damage, and four trace protein urine joint test were higher and clinical promotion.
