吻合器痔上粘膜环切术治疗内痔356例总结

目的总结吻合器痔上粘膜环切术(procedure for prolapse and hemorrhoids,PPH)治疗内痔的临床疗效。方法对2006年3月至2012年3月间所实施PPH356例的手术时间、并发症及疗效进行分析。结果本组手术时间平均25(15～35)min。效果优良者329例(92.4%)。术后肛门明显疼痛14例;并发肛缘水肿9例,术后出血2例,吻合口狭窄2例。无肛门失禁及尿道或阴道壁损伤病例。结论 PPH具有手术时间短、出血少、不破坏肛门结构、疼痛轻、恢复快、复发率低等特点,是治疗Ⅲ度以上内痔或内痔合并出血的较有效方法。

Abstract： Objective To summarize the clinical effects of the method of procedure for prolapse and hemorrhoids （PPH） in treating hemorrhoids. Methods The indications, operation time, complications and curative effects were retrospectively analyzed in 356 patients undergone PPH from March 2006 to March 2012. Results The operation time wasl5 - 35 minutes （25 minutes in average）. There were 329 cases （92.4%） got excellent results. The main complications included obvious postoperative anus pain （14 cases）, edema of anus margin （9 cases）, postoperative hemorrhage （2 cases） and stoma stricture （2 cases）. No anal inconti- nence, urethra, or vaginal wall damage occurred. Conclusion The method of PPH has advantages of shorter operation time, less bleeding, less pain , no destroy of the anus structure, quicker recovery and lower recur- rence, which is recommended as an effective method in the treatment of the third degree or above hemorrhoids with or without bleeding.
