孕妇血清妊娠相关蛋白-A及超敏C反应蛋白水平变化与子痫前期的关系研究

Expression and Significance of Pregnancy Associated Plasma Protein-A and Hypersensitive C-reactive Protein in Serum of Preeclampsia

目的:探讨妊娠相关蛋白-A(PAPP-A)及超敏C反应蛋白(hsCRP)在子痫前期发病中的作用。方法:选择年龄、孕产次相匹配的轻度子病前期(MPE组)、重度子病前期(SPE组)及正常妊娠孕妇(对照组)各30例。检测其血清PAPP-A和hsCRP水平,分析二者与相关临床资料的相关性。结果:①SPE组的新生儿出生时平均体重、Apgar评分、胎盘重量均显著低于对照组,差异有统计学意义(P<0.05),小于胎龄儿发生率、新生儿窒息率明显高于对照组,差异有统计学意义(P<0.05)。②对照组、MPE组及SPE组血清PAPP-A和hsCRP水平依次增高,两两比较,差异均有统计学意义(P<0.05)。③3组血清PAPP-A与hsCRP水平均呈明显正相关(P<0.05);3组孕妇血清hsCRP水平与HCT均无明显相关性(P>0.05);MPE及SPE组血清PAPP-A和hsCRP水平与舒张压及24小时尿蛋白定量均呈明显正相关(P<0.05)。结论:PAPP-A及hsCRP水平升高可能是子病前期发病的重要因素,二者可能共同参与子病前期发病。

Objective：To investigate the expression and alteration of protein-A（PAPP-A） and hypersensitive C-reactive protein（hsCRP） in serum of preeclampsia and to discuss their roles in the pathogenesis of pre- eclampsia. Methods...The expression of PAPP-A and hsCRP in serum of 60 women with PE were tested, who were divided into mild group （n =30） and serious group（ n =30） ,and 30 healthy pregnant women were cho- sen as control. Furthermore, clinical and biochemical parameters were analyzed for their correlations. Re- sults ~ ~The mean fetus birth weight, Apgar score, placenta weight were markedly lower in SPE group than those in control group （P 〈 o. 05）, also the incidences of small gestational age fetus and neonatal asphexia were higher in SPE group than those in control group （P 〈 0.05）. （~The levels of serum PAPP-A in control group, mild and severe PE groups increased gradually, there were significant differences among the groups （P〈0.05）. ~The serum PAPP-A were positively correlated with the serum hsCRP expressions in three groups （P〈0.05）. While there were no correlation between the level of serum hsCRP and HCT in the three groups （ P 〉 0.05）. The serum levels of hsCRP and PAPP-A in mild and severe PE group were positively cor- related with the diastolic pressure （DP） and the 24 hours udne protein （P 〈 0.05）. Conclusions：The in- crease of PAPP-A and hsCRP in serum may be an important elements involved in the pathogenesis of pre- eclampsia, and especially for their coactions.
