昆明市全科医生临床技能操作培训情况调查

Situation of Training on Clinical Skill Operation for General Practitioners in Kunming City

目的了解昆明市全科医生临床技能操作培训情况,为今后的培训方向提供依据。方法采用问卷调查的方法,对参加昆明市第九期全科医生培训的115名学员关于临床技能操作的掌握和需求现状等进行分析,并对其临床理论及技能操作成绩进行查询。结果培训后学员对临床技能操作完全和部分掌握的均在50%左右,同时50%以上的学员对其培训有非常强的需求。学员认为操作模块实训教学方式非常好的有65人(占56.5%);认为视频、面授、专家讲座相互结合教学方式非常好和好的共114人(占99.1%)。学员基本都能顺利通过技能操作考试,各考试科目(除胸部检查外)的及格率均在95%以上。17项临床技能操作考试科目中,鼻饲和问诊的优秀率最高,均为53.9%(62/115);四项穿刺的优秀率最低,为37.4%(43/115)。结论各社区卫生服务中心应加强临床操作技能的培训,尤其是一些技术性强、专业性高的临床技能操作。建议采用以视频、面授、专家讲座及临床技能操作模块实训等相结合的教学方式。

Objective To learn the training condition of clinical skill operation for general practitioners in Kunming city so as to provide a basis for future training. Methods The questionnaire survey was used among 115 general physician （GP） trainees who had participated in the ninth term of clinical skills training. The conditions of clinical skill mastery and the demand by them were analyzed, and their conditions of clinical theory knowledge and clinical skill performance were also exam- ined. Results About 50% of the trainees could completely or partly master the clinical skill, and 50% of them had a great de- mand for the training. There were 65 trainees （56. 5% ） who believed the real practice of operating modules was very good; and 114 （99. 1% ） held that the combined education of video, personal instruction, and experts＇ lecture was quite good. All the trainees could pass the exam on skill practice, and the passing rates of every subjects （expect for chest examination） were more than 95%. Among clinical skill operation test of all the 17 items the excellence rates of nasogastric garage and inquiry were the highest, being 53.9% （62/115） ; and the excellence rate of four sorts of puncture was the lowest, being 37.4% （43/115）. Conclusion Clinical skill training, especially those skills with stronger technology and higher professionalism, should be en- hance for general practitioners in community health service centers. It is suggested that a combined teaching mode consisting of video, personal instruction, experts＇ lecture, and really clinical practice of operating modules should be employed.
