窦性心律震荡在心血管疾病中的临床应用分析 被引量:2

摘要 目的观察窦性心律震荡(HRT)现象在不同心血管疾病中的变化并评价其临床应用价值。方法选择我院检测24h动态心电图(Holter)并发室性期前收缩的心血管疾病组:包括冠心病组、高血压组、心脏瓣膜病组、扩张性心肌病组、老年慢性心力衰竭(心衰组),各40例;选择有室性期前收缩而排除心血管疾病的40例作正常对照组。根据Holter记录资料,计算HRT的两个参数:震荡初始(TO)震荡斜率(TS),比较心血管疾病组与正常组HRT各指标的差异,对不同心功能分级的心衰组按HRT不同分级归类分析。结果各疾病组TO和TS与正常对照组比较,差异均有统计学意义(P<0.05)。不同HRT分类心功能Ⅱ级例数与Ⅲ、Ⅳ级比较,差异均有统计学意义(P<0.05);Ⅲ、Ⅳ级比较,差异无统计学意义(P>0.05)。结论心血管疾病患者存在HRT减弱或消失现象,HRT可用于心血管疾病的病情预测和预后的判断。 Objective To observe the sinus heart rate turbulence(HRT) phenomenon in various cardiovascular diseases and to evaluate changes in the value of its clinical application.Methods hospital Holter test 24h(Holter) complicated by ventricular premature cardiovascular disease groups:including coronary heart disease group,hypertension group,valvular disease of the heart group,dilated cardiomyopathy group,chronic heart failure(CHF group) of 40 cases,choose PVCs to the exclusion of 40 cases of cardiovascular disease as the normal control group.According to Holter recording data to calculate the HRT of two parameters:the initial shock(TO) turbulence slope(TS),cardiovascular disease compared with normal HRT group differences in various indicators of cardiac function in heart failure of different groups according to HRT different hierarchical classification analysis.Results The disease groups T0 and TS compared with control group,the difference was statistically significant(P0.05).Conclusion HRT patients with cardiovascular disease,weakening or disappearance of the phenomenon,HRT can be used to predict cardiovascular disease and prognosis.
