广州地区阴沟肠杆菌中质粒介导喹诺酮类耐药基因的检测及分析 被引量:1

摘要 目的了解广州地区临床分离的阴沟肠杆菌的耐药现状,以及质粒介导喹诺酮耐药(PMQR)基因qnrA,qnrB,qnrC,qnrS,qepA,aac(6')-Ib-cr的流行情况。方法纸片扩散法进行药敏试验;PCR法扩增PMQR基因并将阳性产物测序分析;分析PMQR基因在环丙沙星(CIP)敏感及耐药株中的分布差异。结果 226株阴沟肠杆菌耐药率75%的药物有5种,其中CIP耐药率62.8%。63株(27.4%)检出PMQR基因,qnrA、qnrB、qnrS、aac(6')-Ib-cr的检出率分别3.1%、7.5%、4.4%和15.0%,有5株存在两种PMQR基因,未检出qnrC和qepA基因。qnrA、qnrB、qnrS及aac(6')-Ib-cr基因在CIP敏感株与耐药株中的分布经χ2检验,χ2分别为2.79,10.87,4.63和23.55,P分别>0.05、<0.01、<0.05及<0.01。结论本地区阴沟肠杆菌呈现多重耐药,qnrA、qnrB、qnrS及aac(6')-Ib-cr基因在广州地区均有流行,且这几种基因与环丙沙星等药物的耐药率有一定相关性。
