肾上腺巨大肿瘤的诊断与治疗(附12例报告)

Diagnosis and treatment of enormous adrenal gland mass (Report of 12 cases)

目的 :探讨肾上腺巨大肿瘤的诊治方法。方法 :回顾性分析 1 2例肾上腺巨大肿瘤患者的诊疗过程。结果 :1 2例患者均诊断明确 ,其中 1 0例施行手术 ,9例成功施行根治性切除术 ,1例因肿瘤广泛粘连无法切除只行活检术 ;另 2例作穿刺活检术。随访 1年以上 ,均生活质量尚好。结论 :全面的术前准备对肾上腺巨大肿瘤十分重要 ;根治性切除术是肾上腺巨大肿瘤的首选方法。

Purpose:To investigate the procedures of diagnosis and treatment about enormous adrenal gland mass. Methods:Clinical data of 12 cases of enormous adrenal gland mass was reviewed retrospectively. Results:10 patients received operation:9 cases radical resection, and 0ne biopsy only. Fine needle biopsy for diagnosis was performed in other 2 cases because they refused the operation or can not stand for operation. The masses size were 6 cm×8 cm×8 cm～20 cm×25 cm×30 cm,weighted 250～3 200 g.The pathological diagnosis was verified in all patients: 4 cases pheochromocytoma(One malignant), 3 cases adrenocortical adenomas(0ne malignant), 2 cases myelolipoma, 1 adrenal ganglianeuroma, 1 undifferential neuroblastoma, 1 metastatic thyroid cancer. All patients with malignant tumors were given radiotherapy or chemotherapy. Over one year follow up showed all patients had a life of good quality.Conclusions:Preoperative preparation in detail was very important, and surgically remove of the tumor remains the first choice of treatment.
