染色体畸变与膀胱移行细胞癌临床病理相关性研究

摘要 目的探讨通过多色荧光原位杂交(Multicolor Fluorescence in Situ Hybridization,M-FISH)检测3、7、9、17号染色体数目畸变情况与膀胱移行细胞癌(Bladder Transition Cell Carcinoma,BTCC)临床分期与病理分级的关系。方法采用3、7、17号染色体着丝粒探针和9p21区带探针对46例BTCC尿脱落细胞进行M-FISH检测,得出其敏感性,并分析染色体畸变与BTCC生物学特性的相关关系。结果①M-FISH的灵敏度为73.90%(34/46)。②M-FISH在肿瘤Ta^T1、T2~T4的阳性率分别为50.00%和84.38%,其阳性率与BTCC临床分期呈正相关性(P<0.05)。③M-FISH在肿瘤G1、G2、G3的阳性率分别为42.86%、80.00%和100.00%,其阳性率与BTCC病理分级呈正相关性(P<0.05)。④M-FISH在复发性BTCC的敏感性为83.33%(10/12)。结论使用M-FISH技术检测尿脱落细胞3、7、9、17号染色体数目畸变在无创性辅助诊断BTCC,分析其浸润深度和恶性程度及对预后复发的监测具有重要的参考价值。 【Objective】 To explore the relationship of clinical stage and pathological grade,which of bladder transitional cell carcinoma bladder transition cell carcinoma(BTCC),with numerical aberration of chromosomes 3,7,9 and 17 probed by multicolor fluorescence in situ hybridization(M-FISH).【Methods】 A total of 46 samples of urinary exfoliated cells,collected from the patients diagnosed with BTCC,were detected by M-FISH using centromeric probes of chromosome 3,7,17 and region probe of 9p21 to obtain the sensitivity of M-FISH,and to analysis of the correlation between chromosome aberration and biological features.【Results】 ① The sensitivity of M-FISH in BTCC was 73.90%(34/46).② The sensitivity of M-FISH in Ta^T1 and T2~T4 BTCC was 50.00%,84.38%,respectively,and was related with clinical stage of BTCC significantly and positively(P 0.05).③ The sensitivity of M-FISH in G1,G2,G3 BTCC was 42.86%,80.00%,100.00%,respectively.And was related with pathological grade of BTCC significantly and positively(P 0.05).④ The sensitivity of M-FISH in recurred BTCC was 83.33%(10/12).【Conclusion】 The M-FISH detecting 3,7,9,17 chromosome aberrationns can play a important role in evaluation of tumor infiltration,differentiation and prognosis of BTCC.
