血管内皮素、基质金属蛋白酶-9与脑梗死相关性研究 被引量:3

摘要 目的通过观察脑梗死患者血浆内皮细胞素(ET)、基质金属蛋白酶-9(MMP-9)水平的动态变化与梗死面积、神经损害的关系,从临床方面探讨二者在脑梗死发展过程中的作用。方法对70例脑梗死患者第三天和病后14d的血浆采用免疫分析法内皮细胞素、采用酶联免疫法测定基质金属蛋白酶-9,进行不同时程和同期神经功能缺损程度、脑梗死体积进行相关分析,并与30例正常人对照。结果 70例脑梗死患者第3天时血浆ET、MMP-9浓度为均显著高于对照组(P<0.05)。脑梗死患者血浆ET、MMP-9水平CTV≥10cm3、与CTV<10cm3比较(P<0.05)。不同程度神经功能受损患者血浆ET、MMP-9水平CSS>30与CSS≤30之间比较(P<0.05)。ET、MMP-9水平呈正相关,(r=0.428,P<0.001)。结论 ET、MMP-9参与脑梗死的病理过程,脑梗死急性期血浆ET、MMP-9水平可作为判断梗死面积大小、病情轻重及预后的客观指标。 Objective The dynamic changes of ET, MMP-9 in patients with cerebral infarction and the relation of ET, MMP-9 leves to infarct size and intensity of neurological deficit, the ET, MMP-9 in plasma were de- tected to investigation the effect of ET, MMP-9 to the time course of in ischemic stroke. Methods By RIA, ET concentration in plasma and by ELISA, MMP-9 concentration in plasma were measured in 70 patients with acute cerebral infarction(ACI), on days 3 and 14 after onset and 30 control subjects. At the same time, canadian stroke scale (CSS) score and cerebral infarction volume (CTV) were also obtained and analyzed. Results Mean plasma ET, MMP-9 concentrations in 70 patients significantly elevated compared with control subjects on day 3, (P 〈 0. 05 ). Moreover Mean plasma ET, MMP-9 concentrations in patients with CTV ≥ 10 crn3 , CSS 〉 30 were signif- icantly higher than those with CTV 〈 10 em3 ,CSS≤30, (P 〈0. 05). ET concentrations was significantly positively correlated with MMP-9 in plasma ( r = 0. 428, P 〈 0. 001 ). Conchmion ET, MMP-9 are involved in the patholog- ic process of cerebral infarction. The concentrations of ET and MMP-9 may be helpful as an objective guide to jud- ging the degree,size of infarction area and the recent prognosis in acute cerebral infarction.
