养阴益心方联合胺碘酮治疗阵发性心房颤动31例临床观察

Clinical Observation on Yangyin Yixin Decoction Combined with Amiodarone for 31 Cases of Paroxysmal Atrial Fibrillation

目的观察养阴益心方联合胺碘酮治疗阵发性心房颤动(房颤)的临床疗效。方法选择阵发性房颤气阴两虚型患者61例,随机分为治疗组31例与对照组30例。对照组予胺碘酮口服治疗,用法及剂量为第1周0.2g,每日3次,第2周0.2g,每日2次,第3周起至研究结束0.2g,每日1次。治疗组在对照组基础上加用养阴益心方。两组均治疗6个月。比较两组的临床疗效、中医证候疗效及证候积分变化。结果治疗组临床疗效总有效率为87.10%,对照组为76.67%,两组比较差异有统计学意义(P<0.05)。治疗组中医证候疗效总有效率为90.32%,对照组为73.33%,两组比较差异有统计学意义(P<0.05)。两组治疗后中医证候积分均较治疗前减少(P<0.05或P<0.01);治疗后两组中医证候积分比较,治疗组低于对照组(P<0.05)。结论养阴益心方联合胺碘酮治疗阵发性房颤气阴两虚型患者,具有良好的临床疗效,并且优于单用胺碘酮治疗。

Objective To observe the therapeutic effect of Yangyin Yixin Decoction （nourishing yin and supplementing the heart decoction） combined with amiodarone on paroxysmal atrial fibrillation （PAF）. Methods Sixty-one PAF patients with dual qi-yin deficiency syndrome were randomized into treatment group of 31 cases and control group of 30 cases. The control group was given amiodarone orally, 0.2g in the first week, three times daily, 0.2g in the second week, twice daily, and 0.2g from the third week, once daily. The treatment group was added with Yangyin Yixin Decoction. Both groups were observed for 6 months. The total effective rate, effective rate of syndrome and syndrome scores were observed and compared. Results The total effective rate of the treatment group and the control group was 87.10% and 76.67%, respectively. There was significant difference in the therapeutic efficacy between groups （P〈0.05）. The total effective rate of TCM syndrome of the treatment group and the control group was 90.32% and 73.33%, respectively, with significant difference between groups （P〈0.05）. The syndrome scores were significantly decreased in both groups after treatment （P〈0.05 or P〈0.01）, especially in the treatment group （P〈0.05）. Conelusion Yangyin Yixin Decoction combined with Aminodarone is effective for paroxysmal atrial fibrillation with dual qi-yin deficiency syndrome, and the effect is superior to that of simple using Aminodarone.
