危重患儿应激性高血糖与肝脏功能的相关性 被引量:2

摘要 目的分析危重患儿应激性高血糖水平变化趋势与肝脏功能变化的时相关系,探讨儿科危重症应激性高血糖时肝脏对血糖的调控机制。方法按照前瞻性对照研究原则,选择符合入选标准的危重患儿63例,根据入室当天的血糖水平分为高血糖组(53例)及正常血糖组(10例)。动态监测血糖、肝功能(ALT、AST、LDH、TG、TC、HDL、VDL)水平。结果应激性高血糖患儿应激3 d时AST及VDL与高血糖相关,而TG与高血糖水平相关。研究病例中死亡8例,均为高血糖组感染性疾病患儿,病死率12.7%;存活与死亡(观察时点仍生存时)的血糖水平差异有统计学意义(P=0.000)。结论危重患儿出现应激性高血糖可能为死亡危险因素;应激发生后需要注意保护肝脏功能,维持足够的能量代谢,平衡机体的代谢稳态,同时需防止应激后继发感染与并发症的发生,促进危重患儿的良好转归。
