癌胚抗原、糖链抗原19-9、C-反应蛋白和铁蛋白联合检测对胰腺癌的诊断价值

Value of Combined Detection of CEA,CA19-9,CRP and Ferritin in Diagnosis of Pancreatic Cancer

目的探讨联合检测血清癌胚抗原(CEA)、糖链抗原19-9(CA19-9)、铁蛋白(Ferr)和C-反应蛋白(CRP)对胰腺癌和慢性胰腺炎的诊断价值。方法分别检测CEA、CA19-9、CRP和Ferr在正常对照组、慢性胰腺炎和胰腺癌患者的含量。结果四项指标单独区分正常对照组和胰腺癌组的诊断能力依次为Ferr、CRP、CA19-9和CEA,Ferr的诊断灵敏度和特异性分别为96.60%和96.20%;区分慢性胰腺炎组和胰腺癌组的诊断能力依次为Ferr、CA19-9、CRP和CEA,Ferr的诊断灵敏度和特异性分别为71.20%和87.50%。利用二元logistic回归分析多项指标联合检测的诊断价值,当Ferr和CEA联合检测时,其曲线下面积为0.866,高于CEA、CA19-9、CRP和Ferr单独检测,诊断的灵敏度和特异性分别为74.60%和91.70%。结论 Ferr和CEA联合检测,可提高区分慢性胰腺炎和胰腺癌的诊断能力。

Objective To explore the clinical value of combined detection of serum carcinoembryonic antigen（CEA）,carbohydrate antigen 19-9（CA19-9）,ferritin（Ferr） and C-reactive protein（CRP） in the diagnosis of pancreatic cancer.Methods The concentration of CEA,CA19-9,CRP,and Ferr in patients with pancreatic cancer and with chronic pancreatitis and normal control were detected.Results When the four markers were used alone to distinguish the normal control group and pancreatic cancer group,the diagnostic value from high to low was in the order of Ferr,CRP,CA19-9 and CEA.The diagnostic sensitivity and specificity of Ferr were 96.60% and 96.20% respectively.The diagnostic value of distinguishing chronic pancreatitis and pancreatic cancer was in the order of Ferr,CA19-9,CRP and CEA.The diagnostic sensitivity and specificity of Ferr were 71.20% and 87.50 % respectively.Binary logistic regression analysis was used to indicate the diagnostic value for combined detection.The combined detection of Ferr and CEA was more sensitive than detection of other markers alone.The diagnostic sensitivity and specificity were 74.60% and 91.70%.Conclusion The combined detection of Ferr and CEA could improve the sensitivity and specificity of diagnosis of pancreatic cancer.
