大面积脑梗死不同时期去大骨瓣减压手术临床效果分析

Analysis on clinical results of bone flap decompression in different periods of massive cerebral infarction

目的探讨大面积脑梗死不同时期行去大骨瓣减压手术后的临床效果。方法对2006年1月—2010年12月,16例大面积脑梗死患者行去大骨瓣减压手术治疗的有关临床资料进行回顾性分析,其中早期(瞳孔等圆未大期)手术6例,中期(一侧瞳孔散大期)8例,晚期(双侧瞳孔散大期)2例。结果 16例中存活15例,晚期手术2例中死亡1例。早期手术组3个月后NIHSS评分及梗死对侧肢体肌力恢复情况明显优于中期及晚期手术组,差异有统计学意义(P<0.05)。结论早期行去大骨瓣减压手术可明显改善大面积脑梗死患者神经功能。

Objective To investigate the clinical results of bone flap decompression in different periods of massive cerebral infarction.Methods From January 2006 to December 2010,the clinical data of massive cerebral infarction in 16 patients with bone flap decompression were retrospectively analyzed,including early（and other parks are not large period of the pupil） surgery in 6 cases,medium-term（period of one side of mydriasis） in 8 cases,advanced stage（bilateral mydriasis period） in 2 cases.Results Among 16 cases,15 patients survived,late surgery in 2 cases and 1 patient died.The NIHSS score and infarct contralateral limb muscle strength recovery in early surgery group after 3 months was significantly better than the mid-and late surgery group,the difference has statistical significance（P0.05）.Conclusion Early bone flap decompression for massive cerebral infarction can significantly improve the neurological function of patients with massive cerebral infarction.
